Hospice Social Worker (MSW) Description

Come Join Appalachian Hospice's Calhoun office. Appalachian Hospice based in Jasper, GA (with offices in Cleveland, Jasper and Blairsville) is a locally owned and operated company. Its owners have successfully operated other Georgia based hospice and home health care providers. Their proven record is founded on putting its patient and employees first. Local ownership means local accountability and control verses being at the mercy of the “corporate” office typically mandating a business model focused on the “bottom line”.

At Appalachian Hospice, we believe in a staff model designed to give its clinical staff time to build a trusting and valued relationship with their patients and the patient's families. We do not push “unrealistic” productivity. We except our staff to provide outstanding clinical care delivered with compassion and respect.

Please consider joining our Calhoun based team today. We are looking for a part-time or full-time Master degreed social worker.

Overview:

The Hospice MSW utilizes a Case Management approach to facilitate comprehensive social work planning and care to maximize the comfort and health of our patients and their families in accordance with the interdisciplinary plan of care and regulatory requirements. Further, you will

Provide direct patient care services, with a reasonable case load, to hospice patients and supportive care to the patient’s family
Participate in an interdisciplinary team consisting of Medical Director, RNs, LPNs, CNAs and other staff to enhance the quality of hospice care
Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient
Maintain all assigned patient clinical records
Participate in company quality assessment and performance improvement programs

Qualifications:

Georgia Licensed Master of Social Work.
Current CPR certification.
Previous experience in hospice.
Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance.
