Purpose of the Role
The Clinical Coordinator provides direct behavioral health services and oversees the coordination, implementation, and continuity of clinical care for youth. This position is responsible for conducting clinical assessments, providing therapeutic interventions, overseeing interdisciplinary treatment planning, coordinating care across internal and external service providers, and promoting continuity of services throughout admission, treatment, and discharge. The Clinical Coordinator collaborates closely with residential leadership to ensure that programming supports therapeutic goals and provides clinical guidance to Child Liaisons and other members of the multidisciplinary team to promote high-quality, trauma-informed, evidence-based care.Primary Areas of Responsibility
I. Clinical Mental Health Services
· Conduct comprehensive mental health assessments in accordance with applicable Ohio requirements and agency standards.
· Develop, implement, and regularly review individualized clinical treatment plans based upon each youth's behavioral health needs, strengths, and treatment goals.
· Provide trauma-informed individual, family, and group therapy utilizing developmentally appropriate and evidence-based therapeutic interventions.
· Provide crisis assessment, intervention, stabilization, and therapeutic follow-up for youth experiencing behavioral health emergencies.
· Develop individualized safety plans and clinical recommendations to address significant emotional, behavioral, or mental health concerns.
· Maintain timely, accurate, and compliant clinical documentation, including assessments, treatment plans, progress notes, discharge summaries, and other records required by agency policy.
· Develop clinical recommendations regarding discharge planning, permanency planning, ongoing behavioral health services, and the most appropriate level of care following completion of treatment.
II. Individualized Service Plan (ISP) and Behavior Intervention Plan (BIP) Oversight
· Oversee the development, implementation, and ongoing review of interdisciplinary Individualized Service Plans (ISPs), and Behavior Intervention Plans (BIPs).
· Ensure ISPs integrate behavioral health, educational, medical, family, legal, residential, and discharge planning needs into a coordinated service approach.
· Ensure service plans are individualized, measurable, realistic, and operationally implementable.
· Review and approve ISP/BIP documentation.
· Monitor progress toward established goals and coordinate revisions to the interdisciplinary plan as the youth's needs change.
III. Multidisciplinary Team Coordination
· Facilitate collaboration among therapists, Child Liaisons, nursing, direct care staff, educational providers, custodial agencies, probation officers, foster placements, families, and community service providers.
· Lead or coordinate multidisciplinary case review meetings to promote integrated treatment planning and continuity of care.
· Promote consistent communication, aligned expectations, and coordinated service delivery across all participating systems.
· Assess overall service effectiveness and identify communication barriers, service gaps, or unmet needs affecting youth outcomes.
· Coordinate referrals to community resources, specialized services, and higher levels of care as clinically appropriate.
· Provide strategic oversight of transition and discharge planning to promote successful continuity of services following residential treatment.
IV. Residential Programming Collaboration
· Collaborate with the Program Coordinator regarding census trends, behavioral patterns, developmental functioning, sensory needs, social functioning concerns, and treatment-related programming considerations.
· Provide clinical consultation regarding group dynamics, environmental supports, and therapeutic interventions within the residential milieu.
· Help ensure residential programming aligns with each youth's treatment goals while supporting a safe, trauma-informed, and developmentally appropriate environment.
V. Child Liaison Leadership & Case Consultation
· Provide clinical guidance and consultation to Child Liaisons regarding case coordination priorities, documentation standards, service planning, and continuity of care.
· Review youth records for completeness, accuracy, regulatory compliance, and documentation quality.
· Assist Child Liaisons in navigating complex systems, interdisciplinary coordination challenges, and high-risk or escalated cases.
· Support problem-solving efforts to address barriers impacting treatment progress, service implementation, or successful transitions.
Qualifications· Active Ohio LSW licensure required.· Experience in child welfare, residential treatment, behavioral health, foster care, or youth services preferred.· Strong understanding of trauma-informed care, multidisciplinary collaboration, and wraparound service coordination.· Excellent organizational, communication, and problem-solving skills.· Valid Ohio driver’s license and ability to pass required background checks.
Pay: From $60,000.00 per year
