We're hiring a Clinical Navigator (Medical Assistant) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs.
Clinical Navigators deliver proactive, patient-centered outreach and follow-through. Depending on program needs, this role includes structured patient check-ins, coaching and goal-setting, care coordination to close gaps in care, and addressing barriers such as access to resources, transportation, or medication refills. Clinical Navigators document patient interactions in the Cadence platform and collaborate closely with the clinical team to escalate concerns and coordinate next steps, ensuring patients feel supported, informed, and engaged throughout their care journey.
This is a full-time, remote position based in the United States. Standard working hours are Monday through Friday, from 8:00 AM - 5:00 PM Pacific Time/9:00 AM - 6:00 PM Mountain Time or 9:00 AM to 6:00 PM Pacific Time

The anticipated compensation range for this role is $22 - $26 per hour, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
