Position Overview:
We are seeking a compassionate and dedicated Hospice Chaplain to join our team. As a Hospice Chaplain, you will provide spiritual and emotional support to patients, families, and staff, assisting with the delivery of high-quality end-of-life care. You will have the opportunity to make a significant impact on individuals facing one of life’s most challenging moments.

Responsibilities:

Provide spiritual and emotional support to patients and their families, respecting their religious, spiritual, and cultural beliefs as their Hospice Chaplain.
Conduct spiritual assessments and provide counseling to individuals and families in hospice care.
Lead or facilitate memorial services, support groups, and prayer services as needed.
Collaborate with interdisciplinary teams to create personalized care plans that address the spiritual needs of the patient and family.
Offer support to staff members and volunteers in coping with the emotional aspects of hospice care.
Maintain confidential records of all spiritual care services provided in compliance with organizational policies and HIPAA standards.
Respond to urgent spiritual care needs and provide on-call services as needed.

Qualifications:

Master of Divinity (M.Div.) or equivalent theological degree.
Previous experience as a Hospice Chaplain required.
CPR and BLS certifications are required.
A valid Illinois driver’s license, auto insurance, reliable transportation, and the ability to travel within the service area are required.
Strong ability to provide spiritual care and counseling to individuals from diverse religious and cultural backgrounds.
Excellent interpersonal skills and the ability to work collaboratively with patients, families, and interdisciplinary teams.
Ability to maintain a high level of professionalism and compassion in challenging situations.
Must be available on a per diem basis.
