Full Job Description

Position Summary:

The Care Coordinator is responsible for coordinating and monitoring services for residents within a group home setting. This role serves as an advocate for residents by developing individualized service plans, connecting individuals to community resources, ensuring compliance with program requirements, and supporting residents in achieving personal, behavioral, educational, vocational, and independent living goals.

Minimum Qualifications:

Must be 21 years of age
Must have valid driver’s license or state identification
Must be able to pass criminal background checks
Bachelor’s degree in social work, Human Services, Psychology, Sociology, Counseling, or related field preferred.
Associate degree with relevant experience may be considered.
Minimum 1–3 years of experience in case management, residential services, behavioral health, developmental disabilities, or social services preferred.
Experience working in a group home or residential setting preferred.
Experience with CARF-accredited programs is preferred.
Knowledge of person-centered planning and case management principles.
Understanding of behavioral health, developmental disabilities, and community support systems.
Strong communication, interpersonal, and advocacy skills.
Ability to maintain confidentiality and professional boundaries.
Strong organizational, problem-solving, and documentation skills.
Ability to manage multiple priorities and meet deadlines.
Ability to de-escalate situations and respond appropriately during crises.
Ability to de-escalate situations and manage crisis interventions.
Proficiency in Microsoft Office and electronic documentation systems.
Ability to work independently and as part of a multidisciplinary team.
Must maintain certification for CPR/First Aide, Trauma-Informed Care.

General Manager

Specific Responsibilities

Person Centered Service Coordination:

Develop, implement, monitor, and update Individual Service Plans (ISPs) in conjunction with the Licensed Social Worker.
Conduct assessments to identify resident strengths, needs, preferences, risks, and goals.
Coordinate services identified within individualized service plans.
Ensure residents actively participate in planning and decision-making regarding their services.
Advocate for residents to ensure access to appropriate services and supports.

Case Management and Care Coordination:

Maintain regular contact with residents, guardians, families, and service providers.
Coordinate services with medical professionals, behavioral health providers, schools, employers, vocational agencies, and community resources.
Monitor resident progress toward established goals and objectives.
Facilitate interdisciplinary team meetings and service reviews.
Assist residents in accessing healthcare, transportation, educational opportunities, employment resources, and social services.
Coordinate discharge planning, transitions, and continuity of care when applicable.

Community Integration and Independent Living:

Promote meaningful participation in community activities and resources.
Assist residents in developing life skills including budgeting, personal hygiene, communication, conflict resolution, self-advocacy, and independent living skills.
Encourage participation in educational, vocational, recreational, volunteer, and employment opportunities.
Support residents in achieving greater independence and self-sufficiency.

Resident Rights and Advocacy:

Protect and promote resident rights, dignity, privacy, and informed choice.
Educate residents regarding their rights, responsibilities, and grievance procedures.
Support self-determination and resident involvement in decision-making processes.
Ensure services are delivered without discrimination and with respect for cultural, linguistic, and individual differences.

Documentation and Compliance:

Maintain accurate, timely, and confidential resident records.
Complete assessments, progress notes, case notes, service plans, incident reports, and required documentation within established timeframes.
Ensure compliance with CARF standards, state licensing requirements, Medicaid requirements, and organizational policies.
Participate in audits, surveys, and accreditation activities.
Maintain documentation in accordance with funding and regulatory requirements.

Risk Management and Crisis Intervention:

Monitor resident health, safety, and environmental risks.
Participate in crisis intervention and behavioral support activities as needed.
Assist in the development and implementation of risk reduction strategies.
Report incidents, concerns, and safety issues according to agency procedures.
Participate in incident reviews and corrective action planning.

Quality Improvement:

Participate in continuous quality improvement initiatives.
Assist with data collection, outcome measurement, and program evaluation activities.
Support Legacy Residential Homes' CARF accreditation efforts.
Attend required trainings and maintain professional competencies.
Contribute to organizational performance improvement and resident satisfaction initiatives.
Perform other duties as assigned.

Pay: $21.00 - $23.00 per hour
