Full Job Description

Location: Cincinnati, Ohio Job Type: Full-Time, Office-Based / In-Office Department: Case Management / Care Coordination

About the Role

We are seeking an experienced Director of Case Management to lead and support our case management team in an office-based setting. This is not a facility-based role — you'll be supervising case managers who coordinate care for clients across the long-term care setting.

The ideal candidate brings a strong background in long-term care case management along with proven leadership and team supervision skills.

Long-Term Care Background Preferred: This role requires a minimum of 3+ years of hands-on case management experience within a skilled nursing facility (SNF), long-term care (LTC), MCO and/ or post-acute rehab setting. That experience will inform your work here, but this position itself is based in our office, not a care facility.

What You'll Do

Supervise, mentor, and support a team of case managers, providing guidance on complex cases and professional development
Monitor length-of-stay, discharge planning, and utilization review processes to support quality outcomes and appropriate resource use
Ensure compliance with a wide variety of health plans (MCO) including documentation standards and timely reassessments
Conduct audits and quality reviews of case management documentation and workflows
Serve as a point of escalation for client concerns related to case management issue and processes
Track and report key performance metrics (readmission rates, length of stay, care plan compliance, etc.)
Support onboarding, training, and continuing education for case management staff
Stay current on regulatory changes affecting long-term care case management and update team practices accordingly

What We're Looking For

Required:

Bachelor's degree in Nursing or Experienced RN w/ associate’s degree. Preferred
Active RN or equivalent clinical license (as applicable by state)
Minimum of 3–5 years of case management experience specifically within a long-term care (LTC), skilled nursing facility (SNF) or post-acute/rehab setting — required
At least 1–2 years in a supervisory, lead, or management capacity
Strong working knowledge MCO processes along with the MDS, PDPM, and long-stay care planning requirements
Familiarity with state LTC survey and inspection processes
Excellent communication, organizational, and conflict-resolution skills
Certified Case Manager (CCM) credential a plus

Preferred:

Experience with utilization review and discharge planning in a SNF or LTC facility
Familiarity with EHR/case management software systems
Demonstrated success improving care coordination metrics and team performance
