Full Job Description

JOB DESCRIPTION:

Under the direction of the Support Services Administrator, the Medical Case Manager (MCM) will utilize a strengths-based approach to service that includes treatment, adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in addressing barriers to adequate healthcare for patients served by Cempa Clinic Services. The goals of MCMs are to ensure that clients living with HIV/AIDS have timely access to comprehensive medical care and social services, adhere to medications, are educated on ways to prevent transmission, and to promote and support client’s independence and self-sufficiency. Duties, responsibilities, and activities may change at any time with or without notice.

Education and Experience Required

Bachelor’s Degree in: Human Services, Psychology, Sociology, Social Work or a closely related field.
One year’s experience in social services assessment and service delivery.
Must pass criminal background check and drug testing.
Valid Driver’s license with clean DMV record and the ability to drive an agency vehicle
Excellent driving skills

Education and Experience Preferences

Master’s Degree preferred in Human Services, Psychology, Sociology, Social Work or a closely related field.
Background knowledge of the medical field, mental health and substance use
Bilingual Spanish/English, preferred

Must have knowledge in caring for patients in different developmental stages throughout the life cycle.
Consistent attendance is required to ensure that Cempa client’s needs are met.
Must be able to handle and maintain confidential and proprietary information using discretion in all matters, and high level of integrity.
Demonstrated ability to successfully work in a team environment with collaborative skills.
Ability to prioritize and organize workload, multi-task, adapt quickly to change and deliver under pressure.
Ability to work in difficult, stressful, and occasionally threatening situations.
Must have strong technology skills, including the ability to work in an electronic medical record system, Microsoft Word and Excel, Microsoft Teams, Outlook, and various web-based platforms.

JOB RESPONSIBILITIES (includes but not limited to):

Ensure service delivery is performed according to Cempa’s mission, and to uphold our standards of excellence.
Adhere to all Cempa policies and procedures.
Collaborate with client, healthcare providers and other team members to develop a comprehensive and unified treatment plan to address client needs.
Provide positive customer relations in dealing with clients/patients, their families, and physicians, members of the healthcare team, referral resources, services agencies, all Cempa staff, and others.
Complete documentation thoroughly and in a timely manner, in the medical record and all reporting systems, as appropriate, for each contact or attempted contact with client.
Participate in team meetings and conferences.
Maintain sensitivity and interest to the unique diversity found in the Chattanooga area, including, but not limited to people of color, diverse cultural and religious backgrounds, and economic levels.
Evaluate clients in crisis to determine appropriate course of intervention and facilitate external referral, when necessary.
Assist in all aspects of Social Work Case Management for clients, including, but not limited to referrals, supporting linkage maintenance and consistency, coordination and reduction of service redundancies, client advocacy, development of psychosocial care plan, and appropriate follow-up as needed.
Collect client data and information using specifically designed formats, in accordance with Ryan White Care Act funding requirements.
Develop and maintain thorough knowledge of referral resources throughout the service area.
Participate in internal quality assurance reviews.
Obtain client’s proof of eligibility documentation and verify client is eligible for Ryan White services, as appropriate.
Educate clients about the importance of participating in their medical treatment and treatment adherence
Assess clients’ eligibility for health insurance through the Health Insurance Marketplace, Insurance Assistance Program and other insurance programs, and assist client in applying for appropriate coverage. Submit required documents to appropriate organizations to ensure maximum insurance coverage for client.
Provide Medical Case Management recertification, service planning, monitoring, and follow up activities based on identified need for each client on caseload annually.
Document recertification of client in appropriate Ryan White documentation systems.
Provide evidence informed interventions to assist clients and the interdisciplinary team in managing symptoms and improving functioning. This may be at the request of healthcare providers in the clinic, as needed.
Monitor symptoms and functioning, response to care plan, and need for revisions to care plan based on readiness to change, social and contextual factors and level of engagement.

Continuing Education

Abide by NASW code of ethics, HIPAA and Cempa Community Care Case Management Standards to ensure a prominent level of professionalism is maintained.
Adhere to policies and procedures for Cempa Community Care and other off-site programs to ensure quality standards are met.
Perform routine self-audits and maintain charts to ensure clients information and eligibility documents are current and accurate.
Communicate regularly with staff about changes, updates, and improvements in service delivery issues or other agency related issues that directly or indirectly impact staff and/or clients.
Attend agency, local, state, and national meetings, conferences, trainings or workshops as needed or required.
Participate in and volunteer for intra and interdepartmental activities/events across the agency.
