Full Job Description

CASE MANAGER - FULL TIME - INTELLECTUAL AND DEVELOPMENTAL DISABILITIES (IDD)

Location: McDonough, GA (travel required through 7 counties)
Job Type: Full-Time
Schedule: Mon-Fri 8am-5pm
Pay: $17.00 per hour

Our client in McDonough, GA is seeking a dedicated Case Manager to join their permanent staff and help support individuals on their recovery journey. Responsibilities

Conduct comprehensive assessments of clients' needs and develop individualized treatment plans in collaboration with the treatment team.
Provide ongoing case management services to clients, ensuring they receive timely and appropriate support throughout their residential program stay.
Assist clients in accessing and engaging with various services, such as medical care, counseling, vocational support, and community resources, to support their recovery journey.
Conduct regular progress meetings with clients to review treatment plans, identify barriers, and set achievable personal growth and recovery goals.
Collaborate with other professionals, including counselors, therapists, and medical staff, to ensure coordinated care for clients.
Maintain accurate and up-to-date client records, documenting interactions, progress notes, and treatment interventions in compliance with organizational and regulatory standards.
Ensure adherence to ethical guidelines, confidentiality regulations, and policies related to client care and case management.
Provide crisis intervention and support as needed to ensure clients' safety and well-being during challenging times.

Qualifications

Bachelor's degree in a social service-related field from an accredited college or university.
One year of experience in a social services-related field.
Clean driving record and the ability to pass a background check and drug test.
Current Auto Insurance
Demonstrated experience in case management, preferably in a substance abuse or mental health setting.
Excellent communication and interpersonal skills to effectively engage and build rapport with clients and the treatment team.
Strong organizational skills, with the ability to manage multiple tasks and prioritize responsibilities effectively.
