Full job description:

The Case Manager at the New Heights Recovery Center is responsible for coordinating and overseeing individual and group case management services and support provided to individuals struggling with behavioral health issues and addiction.

Education Requirements: High School Diploma or GED required, Bachelor's degree in Human Services preferred.

Experience Requirements:

Preferred 3+ years of experience in addiction recovery or behavioral health settings, with a strong understanding of case management and community resource connections.
1 year of Case Management experience preferred.

Licensure Requirements:

CDCA license and QMHS III required.
CPR certification required, employer training available.
Must have valid driver's license, acceptable driving record, and be deemed insurable through New Height's insurance carrier.

Job Responsibilities:

Provide assessments to determine the needs of clients seeking behavioral health and addiction recovery services.
Develop individualized treatment plans in collaboration with clients, healthcare providers, and other stakeholders.
Coordinate care and services provided to clients, including therapy, medication management, and group therapy.
Monitor and evaluate clients' progress towards treatment goals and make adjustments to the treatment plan as needed
Advocate for clients to ensure they receive appropriate care and support from healthcare providers and community resources.
Provide education and support to clients and their families about behavioral health issues, addiction recovery, and coping strategies.
Maintain accurate and up-to-date documentation of client interactions, treatment plans, and progress reports.
Collaborate with a multidisciplinary team of healthcare professionals, including clinical and medical teams.
Stay informed about current trends and best practices in behavioral health and addiction recovery services.

Knowledge Requirements:

Excellent communication and interpersonal skills, with the ability to collaborate effectively with a multidisciplinary team.
Knowledge of evidence-based practices in behavioral health, and a commitment to ongoing professional development.
Demonstrated commitment to cultural competence, diversity, equity, and inclusion in clinical practice and program development.
Strong analytical skills with the ability to drive innovation and continuous improvement in addiction recovery services.
Ability to read and interpret written information, write clearly and informatively and edit for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings.
Working knowledge of Microsoft Office Suite (Word, Excel, Powerpoint, OneNote, Calendar, Outlook, and Teams).
Working knowledge of Electronic Medical Records (EMR) systems.
Knowledge of addictions and mental health complications.
Knowledge of community health resources that support life improvement in physical, emotional, mental and spiritual health, economic security, employment, positive social and family interdependence and positive cultural identity including:
Knowledge of referral procedures and admittance criteria for health care and community providers.
Ability to network and collaborate with healthcare and other social and cultural agencies that can provide services to the patients and families.
Knowledge and competency in problem solving, stress management, ethics and team building.
Knowledge and competency in Behavioral Health and substance Use Disorder diagnoses and related symptoms.
Knowledge and competency in crisis intervention, de-escalation, and calming techniques.
Knowledge and competency in risk factors for suicide/homicide.
Knowledge and competency in risk assessment and safety planning.
Knowledge of alcohol and other drugs that includes:
Alcohol and addictive drugs and their physical, emotional, intellectual, and spiritual impact on the individual.
Alcohol and addictive drugs and their impact on the family.
Ability to determine if a medical emergency exists and to take appropriate action, when necessary.
Knowledge of emergency procedures used in case of alcohol and/or drug overdose.
Knowledge of the stages of alcohol and other drug withdrawals and ability to take appropriate action at each stage.

Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team.
