Full Job Description

WHO WE ARE: Since 1936 Central Community House (CCH) has been a source of support and place to gather for children, teens, families and seniors who live, work, and socialize in the Near East and South Side neighborhoods of Columbus. As a full service community center, neighbors of all ages come together to learn, play, share their gifts and find assistance when needed.

WHAT YOU’LL DO: As a Case Manager in CCH’s Strong Families Program you will be a part of a team of talented and passionate professionals focused on engaging and empowering individuals and families experiencing barriers to stability and/or self-sufficiency. You will provide comprehensive case management, support disbursement of crisis assistance funds, provide referrals and resources, and collaborate with partners in the community.

One of your greatest priorities will be developing, reviewing, and updating case plans with your assigned clients. This begins with a thorough intake process where you will gather information, assess unique needs, and work collaboratively with clients to identify their goals. You will also collaborate with others in your department to plan and implement of a variety of social events, educational opportunities and skill-building workshops. Examples may include parenting classes, workshops on topics related to mental health or personal budgeting, family “field trips”, youth groups, etc. An existing knowledge of community resources, excellent organization and communication skills, and a strong ability to empathize are essential in this role in order to ensure clients feel supported, empowered, and connected.

WHAT'S REQUIRED:

Bachelor’s Degree in Social Work, Counseling, Psychology, or related field required. LSW preferred.
Minimum of one year of case management experience
Working knowledge of local social services, community resources and assistance programs
Strong ability to independently manage time and tasks, excellent relationship-building skills, high-energy, and willingness to work both on teams and independently
Satisfactory background check
Must possess a valid driver’s license and provide proof of insurance

KEY RESPONSIBILITIES:

1) Coordinate care and services for families assigned to caseload, including but not limited to:

Manage weekly communication with all clients (in-person, phone/email, and/or home visits)
Develop a personalized case/care plan that addresses clients’ unique needs in the areas of education, health and wellness, safety, housing, basic needs and childcare
Assess eligibility for available material assistance
Provide referrals to community partners (e.g. food pantries, workforce development, PIPP, etc.)

2) Maintain accurate and timely records of all client interactions, progress towards goals, and other programmatic data, as required by grants, funders, etc.

3) Plan and support community events, ensuring alignment with the goals and requirements set by funders

4) Foster a supportive and welcoming environment for community members from diverse backgrounds by demonstrating empathy, patience, and strong communication skills

5) Support and attend community events hosted by CCH, participate in professional development and educational activities, and collaborate with staff across all departments to advance the mission of CCH
