Full Job Description

We are seeking a dynamic and compassionate Home Care Outreach Specialist to serve as a vital link between healthcare providers, patients, and community resources. This role is designed for energetic professionals who are passionate about improving patient outcomes through proactive engagement, education, and support. The ideal candidate will possess strong communication skills, a comprehensive understanding of healthcare systems, and experience working with diverse populations. Your efforts will help facilitate seamless transitions of care, enhance patient satisfaction, and promote access to essential home-based health services.

Duties

Conduct outreach to patients and their families to assess needs and provide education on available home care services
Collaborate with hospital teams, social workers, and community organizations to coordinate discharge planning and follow-up care
Identify eligible patients for home health programs through intake assessments and social service referrals
Educate patients about their health insurance options, managed care plans, and coverage benefits related to home care services
Maintain detailed documentation of outreach activities within EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems in compliance with HIPAA regulations
Facilitate communication between healthcare providers, patients, and caregivers to ensure understanding of medical terminology, discharge instructions, and care plans
Assist in utilization review and management processes by providing relevant patient information and supporting documentation for appropriate resource allocation

Experience

Prior experience in hospital or acute care settings such as ICU (Intensive Care Unit), emergency medicine, or Level I/II trauma centers is highly preferred
Demonstrated knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding systems, and utilization management processes
Experience working with diverse cultural backgrounds and bilingual skills are advantageous for effective client communication
Background in social work or behavioral health enhances ability to address complex patient needs comprehensively
Familiarity with EMR/EHR systems, Microsoft Office applications, and HIPAA compliance standards is essential
Proven ability to coordinate discharge planning, case management, or utilization review within a hospital or hospice environment is highly desirable
Strong interpersonal skills with experience in patient service roles that require active listening and clear communication
