Position Summary

The LPN/RN is responsible for providing hands-on resident care / nursing support / and service] in accordance with the individualized care plans, facility policies, and current state and federal guidelines. This role plays a key part in ensuring the comfort, safety, and well-being of our residents.

Key Responsibilities

● Provide [clinical / dietary / supportive] services that meet residents’ needs and care goals

● Follow all care plans and physician orders accurately

● Document services provided per policy and regulatory requirements

● Communicate clearly with residents, families, and team members

● Maintain a safe, clean, and compliant work environment

● Report changes in resident condition or concerns to appropriate supervisor

Qualifications

● High school diploma or equivalent

● Active LPN or RN license in good standing with Ohio Registry

● Previous experience in a senior care setting preferred

● Strong communication, teamwork, and reliability

● Ability to meet physical demands of the role
