Job description:

Pearl's Hope Home Health Care is looking for a Clinical Director / Director of Nursing (DON) to lead our clinical team across our offices in Columbus and Dayton, Ohio. Home health experience is required. This position will report directly to our VP of Operations.

The Clinical Director / DON is responsible for direction and management of the Pearl’s Hope clinical team and the quality of medical care provided to our patients. This person is an active member of the management team and is responsible for the medical oversight of the regulatory and clinical aspects of our program.

Primary Duties Include

Manage all aspects of the clinical team (RNs, LPNs, etc.) in providing home healthcare services for skilled and non-skilled homecare. Responsibilities span planning, development, organization, and ongoing evaluation of the Pearl’s Hope clinical team and operations in accordance with accepted HCBS requirements, Medicare Conditions of Participation, state licensure requirements, the policies and procedures of the organization, and any other applicable statutory requirements. This also includes driving and/or supporting search and hiring processes and performance management for clinical staff.
Oversee services provided to eligible individuals requesting services to ensure care needs of all patients are met through the supervision of clinical staff (both full-time and part-time).
Maintain and demonstrate a working knowledge of Medicare and Medicaid HCBS requirements, including but not limited to Medicare Conditions of Participation, agency licensure requirements, and accrediting body regulations so that the organization is compliant with accepted standards of practice.
Coordinate efforts with attending nurses to conduct visits to ensure that all nursing visits are delivered in a timely fashion.
Conduct quality checks on nursing charts and documentation as needed, including but not limited to start of care and recertification OASIS assessments, supervisory visits, and bi-annual hands-on skills assessments. Administer corrective action and reeducation as needed.
Support management of clinical staff visit schedules to ensure recertification periods, start of care openings, etc. are conducted in a timely manner complying with all statutory requirements and company policies.
Oversee the Quality Assessment and Performance Improvement (QAPI) Program and ensure that process improvement, quality assurance and utilization review and evaluation of the organization is in place.
Support and advise on authorization denials and reductions in service for appeals as needed.
Support the refinement, development and implementation of policies and procedures relevant to the oversight of all homecare services, contracts, and maintenance of standards relative to accreditation and licensure for HCBS, Medicare, Medicaid, and any other applicable regulatory / accreditation bodies.
Ensure confidentiality of all medical records and personnel files in accordance with agency and HIPAA guidelines.
Exercise independent judgment, critically analyze situations and make recommendations using appropriate problem-solving skills.
Maintain an interest in and a sympathetic, compassionate attitude toward caring for patients. Promote a culture committed to care excellence and high-quality service/
Maintain a professional attitude and demeanor and communicate effectively, courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers and all others. Interact with all staff in a positive fashion supporting the mission and vision of the organization.
Collaborate with cross-functional Pearl’s Hope teams, including but not limited to intake, care coordination, scheduling, and finance to confirm and schedule the opening of new patients and cases.

Skills and Qualifications

Required

3 – 5+ years prior experience in home health care
3 – 5+ years prior experience managing RNs
3 – 5+ years prior experience working in Electronic Medical Record (EMR) software systems
Familiarity with and ability to manage to Medicare Conditions of Participation
Licensed RN
Basic proficiency with Microsoft Office tools (Outlook, Excel, PowerPoint, Word)
Own a vehicle and able to visit client homes if needed
Fluent in English
Excellent oral and written communication skills
Strong attention to detail

Preferred

Experience with non-skilled Medicaid services and requirements (e.g., home health aide, waiver, DODD)
Experience using Alora EMR
Advanced proficiency with Microsoft Office tools (Outlook, Excel, PowerPoint, Word)
