Full Job Description

Description:

The APDS Prevention Facilitator will:

Facilitate Domestic Violence Education Class(s) according to APDS curriculum, standards, and guidelines
Provide one-on-one individual counseling/education sessions for each customer enrolled in group/class
Maintain up-to-date files in compliance with APDS standards for each customer enrolled in group/class
Complete all required paperwork within designated timeframes and at the APDS standard of excellence
Provide professional, on-going, timely correspondence regarding customer progress with authorized contacts for each customer enrolled in group/class
Services emphasize: engagement, skill building, behavioral supports, attendance and retention strategies, case management, and systems navigation.
The position explicitly does not include psychotherapy, diagnosis, or clinical treatment and operates fully within the scope of practice of an RA, OCPSA, CDCA, LCDC II, .

Key Responsibilities:

Conduct Screenings: Perform comprehensive screenings to evaluate the needs, substance use history, and readiness for change of individuals in the program.
Develop Prevention Plans: Create individualized prevention plans that outline goals, strategies, and interventions tailored to each client’s unique needs.
Facilitation: Facilitate individual and group education sessions to support clients in addressing anxiety/anger challenges, conflict resolution, and behavioral issues.
Implement Evidence-Based Practices: Utilize the Duluth Model and other evidencebased treatment modalities, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and trauma-informed care, to support client rehabilitation.
Collaborate with Stakeholders: Coordinate with National Center for Urban Solutions, Franklin County Courts, Franklin County Children Services, correctional facilities, and external partners to ensure continuity of care and appropriate service delivery.
Documentation and Reporting: Maintain accurate and timely electronic health records of client interactions, treatment plans, progress notes, and other required documentation.
Program Development: Assist in the development and enhancement of prevention programs, ensuring they are aligned with the latest research and best practices.
Client Advocacy: Advocate for client needs and facilitate access to resources that support their reintegration and well-being.

Prevention & Early Intervention Services

Provide prevention focused and early intervention support services to students and families.
Deliver social emotional skill building and behavioral support activities (individual and group).
Implement evidence informed, non clinical interventions that promote engagement, self regulation, and positive behavior.
Support attendance, participation, and retention through structured interventions and follow up.

Student, Family & Community Support

Provide case management, care coordination, and resource linkage.
Support families with systems navigation, advocacy, and access to community resources.
Participate in crisis support and stabilization, including referrals to licensed clinicians when clinical needs are identified.
Collaborate with school personnel, APDS staff, and community partners to support student success and well being.

Other Responsibilities:

Participate in weekly Quality Assurance and/or Peer Review sessions
Participate in regular supervision sessions as scheduled
Participate in All Staff Meetings and other meetings as assigned
Maintain agreed upon required level of daily productivity
Participate in data collection and reporting processes and activities as required
Contribute to on-going program and curriculum enhancement and development
Develop strategies and practice techniques which maintain and improve the retention of customers at APDS • Be aware of and abide by all APDS policies & procedures
Serve as a substitute facilitator for other APDS programs as qualified and at request of supervisor • Participate in minimum of sixteen (16) hours of training (internal or external) related to job responsibilities each year of employment at APDS
Participate in and volunteer to assist with other APDS activities and events regularly
Other duties/responsibilities as assigned

Documentation & Compliance

Maintain accurate, timely, and compliant documentation of support services provided.
Ensure documentation language reflects supportive and preventive services, not therapy or clinical treatment.
Adhere to confidentiality requirements, including FERPA, HIPAA (as applicable), and APDS policies.
Participate in data collection, reporting, and quality assurance activities.

Organizational Responsibilities

Participate in scheduled supervision and staff meetings.
Maintain required productivity and documentation standards.
Engage in ongoing training related to prevention, youth development, and supportive service delivery.
Support APDS programs, events, and initiatives as assigned.
Adhere to all APDS policies, procedures, and ethical standards.
Perform other duties as assigned within role scope.
This position does not provide psychotherapy, counseling, or diagnosis. All services are prevention , intervention , and support focused only. Any identified need for clinical mental health treatment must be referred to appropriately licensed professionals.

Requirements:

Associate’s degree in social work, or a related field or a minimum of 3-5 years of experience in clinical practices, case management, or a related role.
Experience working in behavioral healthcare settings or treatment programs.
Current Certification/Licensure/Education which qualifies individuals to perform prevention facilitation services.
Familiarity with Duluth and/or EMERGE Models of DV intervention.
Familiarity with trauma-informed care and cognitive-behavioral interventions.
Familiarity with the American Society of Addition Medicine Criteria (ASAM)
Two (2) years of experience in group or individual counseling and/or facilitation
Excellent verbal and written communication skills, including ability to communicate with people of diverse backgrounds
APDS is a drug free workplace and employment at APDS is contingent upon the results of applicant’s drug test • Employment at APDS is contingent upon background check report(s) regarding applicant Skills and Competencies
Bilingual Spanish Preferred
Experience working within the criminal justice system or with justice-involved individuals.
Proficiency in electronic health record (EHR) systems and clinical documentation.
Efficient use of computer word processing programs, email, and internet
Excellent verbal and written communication skills.
Ability to work independently and collaboratively with a team.
Proficiency in Microsoft Office and other relevant software applications.
Demonstrates maturity and professionalism within interpersonal relationships
Effective independent time management and prioritization skills
Experience working with children, adolescents, families, or community based populations.
Strong communication, organization, and documentation skills.
Ability to work collaboratively within multidisciplinary teams.
Ability to meet APDS background check and drug free workplace requirements.
