Home Health Nurse (RN & LPN) – Flexible Opportunities Across Columbus, Ohio

Position Type: Full-Time | Part-Time | PRN

Schedule: Flexible scheduling based on availability and patient needs

Position Overview

We are seeking dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to join our team. In this role, you will provide skilled nursing care, support patient independence, and collaborate with interdisciplinary teams to ensure optimal outcomes.

Assignments vary based on patient needs and may include short visits or extended care shifts.

Responsibilities

Provide direct patient care in home settings based on individualized care plans
Perform comprehensive assessments (RN) and focused assessments (LPN)
Administer medications and treatments as prescribed
Monitor patient status and report changes to physicians and care teams
Educate patients and families on disease management and care techniques
Maintain accurate and timely documentation
Ensure compliance with all state, federal, and agency regulations

Patient Care May Include

Post-operative care

Chronic disease management (e.g., diabetes, CHF, COPD)zu

Seizure Management

Wound care and dressing changes

Medication administration and education

Pediatric or adult skilled nursing

Geriatric care and support with ADLs

Ventilator/tracheostomy care (for experienced nurses)

Qualifications

RN: Active Registered Nurse License

LPN: Active Licensed Practical Nurse license

Both:

Strong clinical and critical thinking skills
Ability to work independently in a home setting
Excellent communication and patient education skills
Valid driver’s license and reliable transportation
Current CPR/BLS certification
