Overview

We are seeking a dedicated and compassionate Licensed Social Worker to join our dynamic healthcare team. Your expertise will help patients and their families navigate complex emotional, social, and practical challenges associated with health conditions, end-of-life care, and recovery. This position offers an exciting opportunity to make a meaningful difference by delivering patient-centered support grounded in clinical practice standards and ethical guidelines.

Duties

Conduct thorough patient assessments to identify social, emotional, and environmental needs, ensuring holistic care planning.
Provide specialized counseling services including child & family counseling, behavioral therapy, mental health counseling, and addiction counseling to support patient well-being.
Coordinate community resources such as housing assistance, financial aid, transportation services, and support groups to enhance patient recovery and stability.
Collaborate with interdisciplinary teams to develop and implement individualized discharge plans that align with medical treatment goals and social work practice standards.
Document all clinical interactions accurately within electronic medical records (EMR) systems while maintaining compliance with HIPAA regulations and medical documentation protocols.
Engage in crisis intervention when necessary to support patients experiencing acute emotional distress or behavioral health emergencies.
Facilitate individual and group therapy sessions utilizing evidence-based approaches like cognitive-behavioral therapy (CBT), motivational interviewing, and applied behavior analysis (ABA).
Assist in clinical research initiatives related to hospice & palliative medicine or behavioral health to advance best practices in social work interventions.

Experience

Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the relevant jurisdiction.
Proven experience working within hospital settings, inpatient units, nursing homes, or hospice environments with a focus on patient care and discharge planning.
Demonstrated expertise in medical social work practice standards, including familiarity with ICD coding systems (ICD-9/10), CPT coding for billing purposes, and Medicare regulations.
Strong background in working with diverse populations such as geriatric patients, individuals with developmental disabilities or autism spectrum disorder (ASD), dementia or Alzheimer's disease care, PTSD care, and pediatric patients.
Proficiency in crisis management techniques along with experience conducting diagnostic evaluations and case management activities.
Knowledge of EMR systems for medical records management coupled with excellent client communication skills for effective social work practice.
Experience working with individuals from various cultural backgrounds while adhering to clinical ethical guidelines and HIPAA compliance standards.
Familiarity with assisted living environments, outpatient clinics, inpatient facilities, home & community care programs, and post-acute care settings is highly desirable.
