FRANKLIN COUNTY GUARDIANSHIP SERVICE BOARD

Job Title: Social Worker

Department: Clinical

Reports To: Clinical Supervisor

Hours: Standard hours 8:00 a.m. – 4:30 p.m.

Position Status: Full time; Permanent; Non-Exempt; Unclassified

ESSENTIAL DUTIES AND RESPONSIBILITIES OF THE POSITION

· Carry a caseload size of up to 40 clients.

· Complete annual biopsychosocial assessments to identify potential client needs, client preferences, cultural and religious views, views on death, and to ascertain any other existing supports involved in the client’s life.

· Develop care plans to identify services and other supports that would benefit the client.

· Refer clients to appropriate treatment and service providers.

· Collaborate with community agencies, including outpatient mental health providers, developmental disability (DD) providers, nursing facilities, and hospitals to assist with care coordination.

· Document case notes and enter them into agency case management system; update client information and care plans.

· Collaborate with the Legal Department and attend and testify in hearings as required.

· Conduct face-to-face visits with clients quarterly; more frequent visits may be necessary based upon acuity of the case.

· Prepare Court annual reports in a timely manner to remain in compliance with Probate Court rules.

· Review all correspondence in a timely manner and follow-up with the appropriate response. Receive and respond to e-mails and phone calls in a timely manner, including consents.

· Researching and locating different community resources for clients.

· Participate in trainings, consultations, and activities that enhance the Board, its mission, and individual professional development.

· Work with and network with other professionals in the area of guardianship.

· Meet regularly with Clinical Supervisor to review caseload and consult on day-to-day and major life decisions regarding clients.

· Make consent decisions as a designee of the Board in accordance with internal policy.

· On-call availability; on-call rotation sign-up by week on a quarterly basis

· Perform daily responsibilities in a hybrid environment.

· Participate in various committees and groups for the betterment of the agency and the population we serve.

· Perform other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES

· Ability to effectively and professionally communicate verbally and in writing to diverse audiences.

· Knowledge of Microsoft Office Suite Products (Word, Excel, Outlook, etc.)

· Ability to interact and maintain effective working relationships with employees in office and any staff in a telework setting as well as others conducting business with the office.

· Ability to be conscious of, and sensitive to, the diversity within the Board’s jurisdiction and ability to interact professionally with this diverse population of people from many different geographic, socioeconomic, religious, racial, and ethnic backgrounds on a regular basis.

· Ability to demonstrate ability to work independently.

· Ability to follow policies and procedures in a satisfactory manner.

· Ability to document thoroughly, objectively, and timely.

· Ability to effectively communicate with families and other social supports involved with clients.

· Working knowledge of local or regional treatment and service providers.

· Knowledge of working with the Probate Court of Franklin County in the area of guardianship.

· Knowledge of working in the community, especially within Franklin County.

· Knowledge of working with developmentally disabled individuals, individuals with substance abuse, individuals with severe mental illness, older adults, or issues involving decreased mental capacity.

· Knowledge of working with and collaborating with other agencies in the provision of care (e.g., physicians, housekeeping, travel, personal hygiene, law enforcement, mental health agencies, adult day care, recreation, attorneys, dentists, etc.).

· Ability to be organized and demonstrate effective and efficient time management skills.

· Ability to maintain a professional appearance and demeanor appropriate for the position and the work environment expected of a representative of the Board.

· Knowledge of common office practices, procedures, and equipment.

· Ability to demonstrate dependability, reliability, and maintain an excellent attendance record.

· Ability to demonstrate patience, objectivity, maturity, effectiveness under stress, initiative, adaptability, and ability to multi-task.

· Ability to effectively communicate with client’s professional and natural supports on a routine basis.

QUALIFICATIONS AND REQUIREMENTS

Candidates must demonstrate a combination of education, experience, and personal characteristics that support the candidate’s ability to perform the duties of the position.

Minimum requirements include:

· A bachelor’s degree in Social Work or a related field.

· Licensed by Counselor, Social Worker and Marriage and Family Therapist Board of Ohio as an LSW.

· One year of experience in social work.

· Must hold and maintain a valid driver’s license, reliable transportation, and automobile insurance.

· At the discretion of the Executive Director, this position may be allowed to telework. In this instance, the employee's alternative work location must be approved by the Director, have a dedicated workspace with suitable privacy for confidential subject matter, and have sufficiently reliable highspeed internet to perform required job duties.

Preferred requirements include:

· Licensed by Counselor, Social Worker and Marriage and Family Therapist Board of Ohio as an LISW or LISW-S.

· Two years of experience as a Social Worker.

· Experience working in the Probate Court with guardianship.

· Experience in direct clinical care working with the following populations: mentally ill, developmentally disabled, older adults, chronically homeless, drug and alcohol addictions, etc.

INHERENTLY HAZARDOUS OR PHYSICALLY DEMANDING WORKING CONDITIONS:

Has exposure to chemical compounds found in an office environment (e.g., toner, correction fluid, etc.); has contact with potentially violent or emotionally distraught persons; works indoor and outdoor; occasionally exposed to heat, cold, and humidity; exposure to biological hazards (e.g., bacteria, viruses, insects, plants, birds, animals, etc.); exposure to hazardous chemicals.

Note: In accordance with the U.S. Department of Labor physical demands strength ratings, this is considered sedentary work.

BOARD EXPECTATIONS OF EMPLOYEE

In completing the duties and responsibilities of the position, the Board expects that the employee will adhere to all Board policies, guidelines, practices and procedures; act as a role model both inside and outside the Board; perform duties as workload necessitates in a timely, accurate and thorough manner and communicate regularly with the Director regarding department issues.

Employees of the Board are at-will employees and serve at the pleasure of the Board. Employees are not in the classified civil service system and are not members of bargaining units.
