Full Job Description

The Behavioral Health Case Manager (Housing – Addiction Services) provides trauma‑informed, recovery‑oriented case management to individuals with substance use disorders who are experiencing homelessness, unstable housing, or unsafe living conditions. This role focuses on stabilizing clients through recovery housing navigation, relapse‑prevention support, community linkage, and coordination of addiction treatment services. The Case Manager works under clinical supervision and collaborates with multidisciplinary teams to ensure safe housing placement, continuity of care, and adherence to state and federal documentation standards.

Essential Duties & ResponsibilitiesAddiction-Focused Housing Stabilization

Assess housing needs, relapse risks, environmental triggers, and safety concerns related to active addiction or early recovery.
Assist clients in accessing shelters, recovery housing, sober living, transitional housing, or independent living options appropriate for their stage of recovery.
Coordinate with recovery housing operators, landlords, and community partners to support placement, retention, and compliance with house rules.
Educate clients on how housing stability supports sobriety, relapse prevention, and treatment engagement.
Support clients in developing independent living skills that reduce relapse risk (budgeting, structure, communication, safety planning).

Addiction Treatment Coordination & Community Linkage

Facilitate referrals to detox, residential treatment, IOP/OP, MAT providers, peer recovery support, and crisis stabilization services.
Coordinate care with clinical staff, counselors, case managers, peer supporters, and medical providers.
Link clients to community resources that support recovery (food assistance, transportation, employment, benefits, sober support networks).
Maintain collaborative relationships with recovery housing programs, shelters, county agencies, and addiction treatment providers.

Recovery Support & Client Engagement

Provide strengths‑based, person‑centered case management aligned with ASAM criteria and addiction recovery principles.
Assist clients in developing individualized housing stability plans that integrate sobriety goals, relapse‑prevention strategies, and treatment compliance.
Offer motivational support, problem‑solving, and early‑recovery skill building within non‑clinical scope.
Conduct home visits, field outreach, and community engagement to maintain contact with clients who struggle with consistency or relapse.
Support clients during transitions such as detox discharge, treatment admission, relapse episodes, or housing changes.

Documentation & Compliance

Complete progress notes within 24 hours following OAC, CARF, and agency standards.
Document housing plans, relapse‑risk assessments, referrals, collateral contacts, and service coordination.
Ensure compliance with 42 CFR Part 2, HIPAA, and confidentiality protocols specific to addiction treatment.
Track housing outcomes, relapse‑related barriers, and service utilization for CQI reporting.

Professional Conduct & Safety

Maintain professional boundaries and trauma‑informed communication at all times.
Follow agency policies, safety protocols, and ethical guidelines.
Participate in required supervision, annual trainings, and competency evaluations.
Respond to crises using de‑escalation techniques and coordinate with clinical staff and emergency services as needed.

Scope of Practice

The Case Manager (Housing – Addiction Services) operates within OAC 5122‑29‑13 and OAC 5122‑29‑30.

Excluded Activities:

Psychotherapy
Clinical assessment/diagnosis
Independent treatment planning
Clinical interventions requiring licensure
Supervision of clinical staff

Minimum Qualifications

High school diploma or GED required; Associate’s or Bachelor’s degree preferred.
One year of experience in addiction treatment, recovery housing, behavioral health, or social services.
Knowledge of SUD treatment, harm reduction, MAT, relapse prevention, and trauma‑informed care.
Ability to work with individuals experiencing homelessness, active addiction, co-occurring disorders, and complex needs.
Valid driver’s license and reliable transportation.

Required Credentials

Minimum: CDCA, CMS, or QBHS eligibility per OAC 5122‑29‑30.

Physical Demands & Work Environment

Exposure to individuals in active addiction, withdrawal, or crisis.
Ability to lift up to 20 lbs, navigate stairs, and work in varied settings.

Pay: $15.00 - $18.00 per hour
