Full Job Description

Overview

Little Bottoms Free Store (LBFS) is hiring a Community Health Worker (CHW) / Care Coordinator to support our grant funded maternal and infant health initiative in partnership with The Ohio State University Wexner Medical Center (OSUWMC). This role strengthens prenatal care access and improves maternal‑infant health outcomes by coordinating bidirectional referrals between LBFS and OSUMC programs.

Because LBFS serves a diverse community—including a significant number of Spanish‑speaking families—bilingual candidates are strongly preferred. The CHW/Care Coordinator will help ensure families receive equitable, culturally responsive support in their preferred language.

This is a grant‑funded position with funding secured through May 31, 2027. Employment beyond this date is contingent upon future funding.

Key Responsibilities

1. Family Engagement & Intake

Conduct intake screenings for all newly registering LBFS families, including:
Pregnancy status and due date
Infant age eligibility
Prenatal care access
Tobacco/e‑cigarette use
Postpartum depression screening
Social determinants of health needs
Provide trauma‑informed, culturally responsive support to families.
Offer safe sleep education, infant wellness information, and maternal health guidance.
Communicate with families in their preferred language whenever possible.

2. Care Coordination & Referral Management

Facilitate warm handoffs and bidirectional referrals between LBFS and OSUMC maternal health programs.
Coordinate with other program Care Coordinators and other services.
Track referral completion and follow‑up to ensure continuity of care.
Assist families with transportation support for medical appointments.
Participate in planning meetings and reviews.

3. Resource Navigation

Connect families to essential baby supplies including diapers, wipes, formula, cribs, car seats, and clothing.
Provide referrals to community resources such as WIC, Medicaid, SNAP, housing supports, and workforce development programs.
Help families navigate insurance, benefits, and social service systems.
Support families in overcoming barriers related to language, transportation, and access.

4. Data Collection & Reporting

Collect and document participant‑level data in alignment with program requirements.
Maintain accurate records in LBFS and program data systems.
Track key metrics including:
Number of families screened
Number of referrals made
Number of referrals completed
Health screening outcomes
Recording the items received by program participants (car seat, crib, diapers, bus pass, etc)
Participate in evaluation and quality improvement activities.

5. Program Implementation & Partnership Support

Support the development of referral workflows, consent procedures, and intake processes.
Represent LBFS in community and clinical partnership settings.
Contribute to continuous improvement of the Thriving Families Program.

QualificationsRequired

High school diploma or equivalent.
Community Health Worker certification.
Experience working with pregnant or parenting families, especially in under‑resourced communities.
Strong interpersonal skills and the ability to build trust quickly.
Commitment to trauma‑informed, culturally responsive care.
Ability to maintain confidentiality and follow HIPAA guidelines.
Comfort working in both community and clinical partnership environments.

Preferred

Bilingual English/Spanish or English (strongly preferred due to community need).
Lived experience reflective of the communities LBFS serves.
Experience in maternal/infant health, social services, or community outreach.
Familiarity with Franklin County’s high‑risk ZIP codes and local resource networks.
Experience supporting families navigating Medicaid, WIC, SNAP, or similar programs.

Work Environment

Primarily onsite at Little Bottoms Free Store in Franklinton.
Occasional meetings at community partner locations.
Thursday evening availability required during LBFS distribution hours and occasional weekends at community events as a staff representative for LBFS.

Employment Term

This position is fully funded through the grant cycle ending May 31, 2027. Continued employment beyond this date is dependent on future funding.
