Full Job Description

Job Overview

Join our dynamic healthcare team as a Registered Nurse and become a vital part of delivering exceptional patient care across diverse home settings. In this role, you will provide comprehensive nursing services, coordinate patient assessments, administer treatments, and support recovery and wellness initiatives. Your expertise will help improve health outcomes while ensuring compliance with healthcare standards and regulations. This position offers an exciting opportunity to work in a fast-paced environment where your skills directly impact patients’ lives and well-being.

Duties

Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
Administer medications and injections accurately following aseptic techniques and medication protocols, including IV infusions and catheter care.
Perform specialized procedures such as suturing, tube feeding management, catheterization, and wound care in accordance with best practices.
Monitor patients’ physiological responses during critical care situations like ICU or trauma medicine, utilizing telemetry and ventilator management skills.
Collaborate with interdisciplinary teams to develop and implement individualized care plans for diverse populations including pediatrics, geriatrics, behavioral health, home health and patients with developmental disabilities.
Ensure compliance with HIPAA regulations by maintaining accurate medical documentation and managing electronic health records (EHR) systems effectively.
Assist in diagnostic evaluations such as laboratory specimen collection

Skills

Extensive experience working with EMR/EHR systems such as Wellsky( previously Kinnser) for documentation and patient management.
Proficiency in performing complex nursing procedures including phlebotomy, ostomy care, sterile processing, and infection control protocols.
Strong knowledge of physiology, anatomy, ICD coding (ICD-10), CPT coding for billing purposes, and laboratory testing procedures.
Skilled in managing diverse patient populations across settings like hospital medicine, outpatient clinics, home & community care environments including long-term care facilities and assisted living communities.
Ability to perform assessments in a home environment setting while demonstrating basic life support (BLS) skills.
Familiarity with specialty areas such as cardiology (cardiac catheterization), infectious disease care, occupational health services, neonatal resuscitation procedures (NRP), and geriatrics including dementia & Alzheimer’s care.
