Full Job Description

The Texas Family Doctor - Crockett Office is currently seeking a new staff member to serve as Patient Care Coordinator. Applicants must, ABOVE ALL, have a passion for caring and helping others. A special focus of this position will be coordinating and performing patient services related to screening, prevention and overall wellness.

The most qualified applicant/s will be detail-oriented, computer-savvy, comfortable with multi-tasking, an excellent communicator, work with a smile, be patient with patients, creative in problem solving, and willing to adapt to innovative ways of providing excellent care to patients of all ages.

A patient care coordinator manages communication between patients, their families, and healthcare providers. They also help patients navigate their healthcare and treatment plans. .

Responsibilities

Assisting patients during and between visits, including with intake questionnaires and health screenings, vitals, medication reconciliation, updating medical history, connecting and coordinating care with specialists and other care facilities
Educating patients about their health issues
Coordinating a program of Annual Wellness Visits and tracking of indicated screenings and preventative care
Performing ancillary testing such as EKG, PFT, and others as indicated.
Tracking and updating of significant Quality Measures to help patients stay healthy

Skills and qualifications

To best qualify for a patient care coordinator position, candidates may possess one of the following certifications:

MA (Medical Assistant)
LVN (Licensed Vocational Nurse)
RN (Registered Nurse)

In addition to any certification/s above, clinical experience and excellent communication skills are paramount for a patient care coordinator. This experience should encompass:

Direct patient care
Clinical assessment and evaluation
Coordination of treatment plans
Knowledge of medical terminology and procedures

A patient care coordinator will be working collaboratively both within and outside the office with a variety of caregivers, healthcare professionals, Physicians, Advanced Practitioners, community resources and other stakeholders in the health of the patients for whom we care. Interpersonal skills, cooperation, and tireless advocacy are very important to be successful in this position. The medical system as a whole can be confusing and frustrating to patients. It is vital that this person in this position serves as a welcome refuge for patients, and is able to advocate strongly on their behalf.

Candidates MUST be reliable, trustworthy, professional and able to work independently with minimal supervision. Salary and benefits are negotiable based on qualifications and experience.

This position will be for traditional office hours Monday through Friday with no weekends scheduled. Most major holidays will be off.
