WV Caring | Full-Time | Arthurdale, WV

Join Our Team

WV Caring is seeking a motivated and detail-oriented Licensed Practical Nurse (LPN) to join our Quality Assurance and Compliance team. This role is ideal for an LPN who enjoys improving quality of care, reviewing clinical documentation, supporting staff, and ensuring regulatory excellence. You’ll work closely with clinical leadership to help maintain the highest standards of hospice care.

Responsibilities

Review patient records for accuracy, completeness, and regulatory compliance.
Audit clinical documentation, including HOPE (Hospice Outcomes and Patient Evaluation) assessments, to ensure accuracy, timeliness, and compliance with CMS requirements.
Monitor compliance with Medicare Conditions of Participation, state regulations, accreditation standards, and organizational policies.
Support Quality Assurance and Performance Improvement (QAPI) initiatives through data collection, chart reviews, and quality monitoring.
Assist with regulatory survey preparation and ongoing survey readiness.
Identify documentation trends and opportunities for improvement, and provide feedback to clinical teams.
Assist with quality reporting, performance metrics, and compliance monitoring.
Collaborate with clinical leaders to improve documentation practices and patient outcomes.
Assist in developing and updating policies, procedures, and educational materials.
Provide education and coaching to staff on documentation standards, regulatory requirements, and quality initiatives.
Perform other related duties as assigned.

You may also want to add this to the preferred qualifications:

Preferred Qualifications

Hospice or home health experience preferred.
Experience with HOPE assessments, hospice quality measures, chart auditing, or regulatory compliance is strongly preferred.
Familiarity with Medicare hospice regulations and CHAP accreditation standards.

Qualifications

Current West Virginia LPN license in good standing.
Minimum of two years of nursing experience preferred.
Hospice, home health, quality assurance, compliance, or case management experience is a plus but not required.
Excellent organizational, communication, and computer skills.
Ability to work independently while managing multiple priorities.
Valid driver’s license and reliable transportation.
