Collaborative Living provides 24-hour residential care and supervision to Oregon people with intellectual disabilities/developmental disabilities who represent the most risk to the public at large, their peers, or themselves. Collaborative Living provides residential care, which includes health and medical care, behavioral support, job and training support, personal care, education, recreation, psychological services, and community integration. Collaborative Living is dedicated to providing the support necessary to maintain the quality of life, achieve the highest possible level of independence and promote social opportunities, which benefit the individuals and the community. Each individual residing in the program has an Individual Support Plan (ISP) that emphasizes community integration, independence, and productivity, has daily implementation goals, and is monitored by county case management. Your Direct chain of command:
- House Manager 2.Area Manager 3.Assistant Director 4. Executive Director. HR is to be contacted for any complaints pertaining to the chain of command that there may be.
- Required shift: this will be a type of float position that is semi flexible. Meaning that from Friday to Tuesday the shift will begin at either 9am to 5pm or 10am to 6pm (depending on the house needs). However, on these days if someone unexpectedly calls out for day, swing, or NOC and no other coworker offers to pick up the shift then the Team Lead will be able to shift their schedule to meet the staffing needs. Their weekend when they are not responsible for the shift coverage will be on Wednesday and Thursday. On the days of Friday, Monday and Tuesday this responsibility for coverage will be shared with the Manager but coverage is decided at the discretion of the Manager.
- Due to this shifting schedule the starting pay for Teams Leads will be $20 per hour with the ability to reach up to and no higher than $24 per hour.
Section 2: Working Conditions:
The work site is in a home environment, which is staffed 24 hours/day, seven days/week. Residential care is provided for adult supported residents. A group home may be “locked” or “unlocked” depending on the security needs of the supported individuals in the homes and the surrounding communities. Kitchens, laundry, and cleaning areas may be locked in some of the group homes based on the individual’s Individual Support Plan or Positive Behavior Support Plan. Furniture and wall decorations in the “locked” homes may be secured to the floors and walls for the safety of the individuals and the staff.
Must be able to work occasional, required overtime to ensure adequate staffing requirements. Work with combative, unpredictable, argumentative, aggressive adults who experience intellectual or developmental disability and who may be a danger to themselves and others daily. Frequent daily lifting up to 75 lbs. involving transferring, positioning, and moving individuals and/or moving program supplies. Frequent daily squatting, bending, and stooping.
Must be able to engage and maintain Oregon Invention System (OIS) approved Protective Physical Interventions (PPIs) for one (1) hour or less. Daily exposure to household and sometimes industrial strength cleaning agents. Exposure to a variety of work settings both within the home(s) and the community; This includes and is not limited to: exposure to activities of daily living that an individual may engage in such as, loud television noises or music, scented perfumes or air fresheners, foul language, discussions involving individuals’ preferences in religion or entertainment materials, posters, books, magazines and clothing that reflect individual preference. Occasional exposure to inclement weather. Must maintain a valid driver’s license and an acceptable driving record as frequent use of a motor vehicle to transport adults throughout the area to and from appointments and desired approved locations. Dress in semi-professional work attire (examples: court, family visits, school, etc.). Must be able to speak, read, and write in English. Must have basic computer skills / be able to utilize the THERAP system for documentation and communication with supervisors and co-workers. Skill in providing clear and concise verbal and written communications. Skill in prioritizing a heavy and often changing workload. Skill in working with individuals who may experience emotional, behavioral, or physical challenges. Skill in resolving conflicts in a professional and timely manner.
Section 3: Duties and Responsibilities
Personnel Management and Supervision -This means assisting your management team with:
- Working across all three shifts to maintain and provide training, and support of the individuals. - Under direction from the management team, provide training, coaching and supervision to employees. - Under direction from the management team conduct staff meetings on all shifts.
- Respond and resolve grievances on a basic level. Under management team direction conducts fact-finding meetings and discuss necessary action with the manager.
- Complete New employee orientation checklists. - On-call for crisis management alternating with management team Client program Management:
- Help aid the management team in ensuring that each client has an up-to-date ISP
- Under management team direction help to complete PFW’s, RITs, and provide input for the behavior support plans. - Ensure all packets and paperwork are in order for ISP meetings under management team direction. - Maintain record confidentiality.
- Work with Managers and employees to ensure that individuals' health needs are met and taken care of. - Review MARs to verify no mistakes, mishaps, etc. Household Management:
- Read, understand, and consistently follow company policies and OARs
- Arrange well- balanced meals and supplies to make such.
- Assign duties and schedules as directed by the management team.
- Aid in the training of evacuation procedures and drills, maintain the reports. All other duties as assigned by the management team.
