Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team as a Staff Nurse. In this vital role, you will provide high-quality patient care in a long-term care setting. Your expertise will support patient recovery, comfort, and well-being through comprehensive nursing interventions. You will work closely with interdisciplinary teams to deliver personalized care plans, ensuring compliance with healthcare regulations and maintaining the highest standards of clinical excellence. This position offers an exciting opportunity to make a meaningful difference in patients’ lives while advancing your nursing career. We are a privately owned Skilled nursing facility! We have do not use agency and we do not mandate for call offs! We also like to utilize Certified Med Techs so our nurses can focus on nursing duties. Responsibilities

Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform care planning.
Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments.
Manage complex care needs such as dementia, Alzheimer’s disease, memory care, and rehabilitation conditions with sensitivity and professionalism.
Operate electronic health record (EHR) systems like Point Click Care to document patient information securely and efficiently.
Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures as required.
Provide wound care, catheterization, tube feeding management for patients with acute or chronic conditions.
Collaborate with physicians and other healthcare professionals in Admissions, rehab, PT/OT/ST, memory care, and discharge planning.
Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and patient information.
Support patients’ emotional well-being through compassionate communication and education on health coaching topics such as nutrition or medication management.
We are open to educating and assisting in the growth of new nurses.
We offer educations in house that can qualify towards CEU's!

Qualifications

Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
Strong knowledge of medical terminology, anatomy, physiology, documentation standards.
Experience with EMR/EHR systems such as Point Click Care is highly desirable.
Competency in performing IV insertions, medication administration—including injections—and basic life support certification.
Familiarity with specialized care areas including dementia/Alzheimer’s care, hospice care, infectious disease management, pain management strategies, and trauma response protocols.
Ability to work effectively with geriatric patients and individuals with disabilities.
Excellent patient assessment skills combined with a compassionate approach to delivering holistic nursing care.
