The Re-entry Case Manager will provide wrap-around services to support clients who experience barriers due to a criminal history. The goal of this position are to assist moderate to high risk formerly incarcerated individuals, who are returning to St. Tammany and Orleans Parishes, in order to help prevent recidivation.
What You'll Do:

Conduct prison in-reach sessions with clients to begin assessing their needs upon their release. Begin to identify and address barriers to community reintegration.

Maintain a caseload of up to 50 clients.

Assist clients with accessing public benefits.
-
Assist clients with locating housing, access to food resources, legal needs, transportation barriers, and make appropriate referrals for needed services.

Navigating community health services, including mental health services and substance abuse treatment.
-
Monitor participants progress and routinely followup with participants.
-
Train participants on development of interview and soft skills through coaching.
-
Assist clients in locating permanent employment.
-
Resume development.
-
Develop and work on long and short term goals.
-
Obtain and verify participants work hours and compliance at transitional employment work sites.
-
Provide appropriate guidance and referrals to achieve educational goals such as high school equivalency/HiSET and other educational pursuits.

Maintain detailed, complete and accurate records of all case management work, client progress and program administration utilizing the designated data-recording tools.

Coordinate with local community partners to meet client needs.

Maintain relationships with Parole Officers of the clients'.

Attend and particpate in staffings.
-
Assist with locating community resources for clients and identifying felon friendly employers.

Moderate Re-Entry Classes.

Assist with problem solving skills.
-
Complete assigned projects and any other duties, as required.

What You'll Bring:
-
Bachelor's degree in Social Work, Criminal Justice, Psychology or similar field preferred.
-
Training in CBT, MRT, Trauma Informed Care, Conflict Resolution, strongly preferred.
-
3-5 years of case management or workforce background in criminal justice or probation and parole.
-
Working with justice involved individuals is highly preferred.

Must have and maintain legal, reliable transportation.

Willing to travel throughout Orleans, St. Tammany, and surrounding Archdiocese of New Orleans service areas as needed.

Must be able to obtain clearance to enter a correctional facility.

Excellent verbal and written communication skills.

Strong organizational and interpersonal skills.

Ability to type a minimum of 35 words per minute.

Knowledge of office procedures, etiquette, and recordkeeping systems.

Proficient in Microsoft Office Suite or similar software, with the ability to learn new or updated systems.

Ability to work collaboratively as part of a team.

Capable of managing assigned tasks from start to completion in a timely and professional manner.

Ability to manage multiple priorities simultaneously.

Detail-oriented with a commitment to producing high-quality work.
