Your Specific Responsibilities:
This is advanced-level work in the Community Health Improvement Planning Branch, promoting, maintaining, and improving individual and/or community health by assisting individuals and communities to adopt healthy behaviors as they relate to substance use prevention, harm reduction, and community education. Complex tasks are performed under limited supervision where general policies may not have been previously established and standard methods may not be available. The position requires an extensive knowledge of community dynamics and an ability to be an effective agent for change. This position serves as the lead for the Post Overdose Support Team.
Incumbent will work to implement deliverables as defined within the Opioid Settlement funding allocation Memorandum of Agreement and serve as the liaison for projects therein. The incumbent will work collaboratively with other Health Educator Consultants and community partners on projects, including any overlap with the Community Health Improvement Plan (CHIP) action plan items that specifically include substance use and/or opioid-related activities. Incumbent will serve as a resource to assist individuals, other professionals, and the community to make policy and systems changes associated with the substance use-related priority areas designated by the Okaloosa County Health Improvement Partnership (OCHIP) workgroups. Incumbent will work collaboratively with OCHIP on substance use and/or opioid-related activities in alignment with opioid remediation uses outlined in Exhibit E of the Opioid Settlement. Examples of OCHIP substance use-related priority areas include, but are not limited to, access to substance use treatment and linkage to care, drug endangered children, opioid overdoses, overdose mortality, substance use prevention, training and education, and harm reduction.
Incumbent will serve as program manager for the Post Overdose Support Team (POST). They will be responsible for overseeing all administrative functions of the POST program, including tracking and monitoring programmatic data, developing quarterly and annual programmatic reports, developing programmatic materials and resources (i.e. rack cards, presentations), and ensuring all programmatic documents are updated and reflective of current procedure, including the Implementation Plan, Training Plan, Program Evaluation, and other programmatic documents. The incumbent will assist with outreach efforts of POST such as targeted outreach events, Naloxone distribution events, community events, and others as identified. The incumbent will complete other data requests and/or reports as applicable from partners and/or the community. They will lead efforts and initiatives involving peer inclusion into POST and peer-related infrastructure needs. They will complete other administrative tasks as identified and/or assigned by the POST Leadership Team or supervisor.
Collaborates with health specialists and civic groups to ascertain community health needs related to substance use, determine availability of services, and support substance use programming efforts. Promotes health discussions in populations such as first responders, law enforcement, substance use treatment providers, pregnant and postpartum women, school-aged youth and/or parents of school aged youth, and community agencies to encourage policy or systems changes related to OCHIP substance use and/or opioid-related priority areas. Mobilizes the community to define and address priority health issue prevention and develops partnerships to address goals.
Promotes OCHIP substance use and/or opioid-related initiatives among community partners, businesses, childcare providers, and individuals, among other relevant partners. Provides direct support to community partners to ensure successful implementation of health promotion strategies and identified best practices. Communicates extensively with persons outside the organization. Tailors communication to the needs of the audience. Represents the organization to the public, government, and other external sources. Provides information to supervisors and fellow workers. Information can be exchanged face-to-face, in writing, or via telephone/electronic transfer. Develops and updates plans, procedures, and deliverables as defined within the Opioid Settlement funding allocation Memorandum of Agreement, including those related to POST and OCHIP group action plan activities related to substance use. Responsible for administrative tasks, including meeting facilitation and action item progress tracking related to the Community Health Improvement.
Conducts activities to increase awareness of substance use and/or opioid-related priority areas. Develops and maintains cooperation between public, civic, professional, and voluntary agencies. Prepares and disseminates educational and informational materials that are linguistically and culturally appropriate. This includes the development of various types of media in alignment with the media campaign development. Develops constructive and cooperative working relationships with others. Provides consultation and expert advice on program components for POST, including technical, systems-related, or process related topics. Translates or explains information in a way that members of the community can more easily understand issues related to substance use. Completes documentation and provides updates on deliverables as agreed upon with the Okaloosa County Board of County Commissioners.
Collects and analyzes data to identify individual and/or community needs prior to planning, implementing, monitoring, and evaluating programs or services designed to improve community health such as POST. Regularly researches best practices related to substance use prevention, social marketing, education, and programming. Informs supervisor of best practices and conducts additional research as requested. Conducts community surveys to ascertain health needs, develop desirable health goals, and determine availability of resources related to substance use and/or opioid-related priority areas. Uses information and data to recognize differences or similarities and sense changes in circumstances or events. Modifies behavior or approach based on this information. Develops multiple approaches for implementing plans in a linguistically competent and appropriate manner. Assists community members to establish systems for implementing best practices. Works with key stakeholders to implement promising practices, best practices, evidence-based practices, or evidence-informed strategies as applicable. Conducts quality improvement activities to improve existing programs and partnerships. Submits reports on a regular basis per grant requirements as applicable. Collaborates with colleagues and/or community partners to ensure correct and timely reporting on all indicators. Completes purchases for programmatic activities/initiatives/projects.
Attends training and maintains competencies. Remains knowledgeable of procedures and technical skills necessary to obtain demographic and community health data on Okaloosa County residents. Investigates best practices to inform, educate, and empower the community regarding public health substance use-realted initiatives. Prioritizes work and uses time wisely. Maintains confidentiality of all information within the health department. Performs emergency duties which may include work in shelters and control centers. Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

Qualifications:
Preferred: Bachelor’s degree or higher in Health Education, Health Promotion, Community Health, Public Health, Social Work, or other health-related field.
A minimum of 6 months of experience in the past 3 years working and/or volunteering in health promotion/health education/social work, and/or leading community health improvement activities required
CHES, CPH, MCHES, CHW, or LCSW preferred
