Physical Therapist Assistant (PTA)

Atlanta, Georgia | Full-Time | Skilled Nursing & Long-Term Care

Position Summary

Our Nursing and Rehab Center is seeking a licensed Physical Therapist Assistant to join our rehabilitation team. Working under the direction and supervision of a licensed Physical Therapist, you will deliver hands-on treatment to short-term rehab and long-term care residents — restoring mobility after hospitalization, reducing fall risk, and helping residents hold onto the independence they have. Our caseload spans post-acute orthopedic and cardiopulmonary recovery, stroke and neurological rehabilitation, wound and contracture management, dementia-related functional decline, and long-term restorative programming.

This is a busy building, and we are honest about that. You will carry a full daily caseload, move between resident rooms, the therapy gym, and the dining and activity spaces, and work shoulder-to-shoulder with nursing, dietary, and social services. In return you get real continuity — you know these residents and their families by name, you see them across weeks and months, and you get a rehab department that backs you up with adequate staffing, working equipment, and supervisors who treat documentation support as their job, not yours.

Key Responsibilities

· Implement resident treatment plans established by the supervising Physical Therapist, including therapeutic exercise, transfer and gait training, bed mobility, balance and fall-prevention work, contracture management, and modalities.

· Deliver care in the therapy gym and at bedside, adapting each session to the resident's tolerance, cognition, and medical status on that day.

· Train residents, families, and nursing staff in safe transfers, positioning, assistive device use, and restorative programs that carry over between therapy sessions.

· Observe, measure, and document resident response to treatment — range of motion, strength, functional mobility scores, pain, endurance — and report changes in status to the supervising PT and the nursing team promptly.

· Complete accurate, timely documentation in the electronic medical record that supports skilled coverage under PDPM, Medicare Part A and Part B, Medicaid, and managed-care requirements.

· Progress or regress interventions within the established plan of care and the PTA scope of practice under Georgia law.

· Contribute to interdisciplinary care planning, Medicare and utilization review meetings, and discharge planning, including home safety recommendations and equipment needs.

· Support the facility's fall-prevention, restorative nursing, and skin-integrity programs and reinforce safe handling practices on the units.

· Follow infection-control, isolation, and outbreak protocols consistently, including PPE use and equipment sanitation between residents.

· Meet productivity expectations while keeping resident dignity, safety, and quality of care ahead of volume — and speak up when the two conflict.

Required Qualifications

· Graduate of a physical therapist assistant program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).

· Current license (or eligibility for licensure) as a Physical Therapist Assistant issued by the Georgia State Board of Physical Therapy.

· Current BLS/CPR certification from the American Heart Association or American Red Cross.

· Working knowledge of PTA scope of practice and supervision requirements under the Georgia Physical Therapy Practice Act.

· Comfort treating a medically complex geriatric population, including residents with dementia, and the patience and warmth that population deserves.

· Competence with electronic medical record documentation and defensible daily-note and minute-tracking standards.

· Ability to pass a background check, fingerprinting, and required health screenings, including drug screen, TB test, and immunization verification consistent with long-term care regulations.

Preferred Qualifications

· One or more years of experience in a skilled nursing facility, subacute rehabilitation, or long-term care setting.

· Working knowledge of PDPM, Medicare Part A and Part B billing, and skilled-therapy documentation requirements in long-term care.

· Experience with post-acute total joint replacement, hip fracture, stroke, and cardiopulmonary recovery protocols.

· Training in dementia care, LSVT BIG, vestibular rehabilitation, wheelchair seating and positioning, or certified exercise expert for aging adults (CEEAA).

· Bilingual in English and Spanish.

· Familiarity with PointClickCare, Casamba, Net Health, or similar long-term care therapy platforms.

Schedule and Location

Full-time, 40 hours per week, based on site at Nursing and Rehab Center in Atlanta, Georgia. Therapy department hours are 7:00 a.m. to 5:30 p.m., with a rotating weekend commitment of approximately one weekend per month and no overnight shifts. Caseload and treatment minutes vary with census and payer mix; expect a full but achievable schedule with built-in documentation time.

Compensation and Benefits

Base pay up to $87,500 annually, commensurate with experience and credentials, plus a $5,000 sign-on bonus. Additional compensation may include shift and weekend differentials, PRN or overtime opportunities, and referral bonuses.

Physical Requirements

· Frequent standing, walking, kneeling, squatting, and bending throughout an eight-hour shift, including extended time on resident units.

· Ability to lift, transfer, and physically support dependent residents and to lift or move equipment weighing up to 50 pounds, using mechanical lifts and assistance per facility policy.

· Sufficient manual dexterity and tactile sensitivity to perform manual techniques, apply modalities, and manage assistive devices safely.

· Visual and auditory acuity adequate to monitor resident status and respond to call lights, alarms, and verbal cues.

· Ability to wear personal protective equipment for extended periods and work safely in isolation rooms.
