Job Summary

We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate and deliver comprehensive hospice care, ensuring patients receive comfort, dignity, and personalized support during their end-of-life journey. Your expertise in clinical case management, patient assessment, and interdisciplinary collaboration will help create a nurturing environment that prioritizes patient-centered care. This position offers an exciting opportunity to make a meaningful difference in the lives of patients and their families while working within a supportive and innovative healthcare setting.

Responsibilities

Conduct thorough patient assessments to evaluate clinical needs, symptoms, and overall condition, utilizing your expertise in physiology knowledge and medical terminology.
Develop individualized care plans aligned with hospice protocols, patient preferences, and regulatory guidelines, including discharge planning and post-acute care coordination.
Manage clinical case management activities by monitoring patient progress, adjusting care strategies, and ensuring timely interventions to optimize comfort and quality of life.
Collaborate effectively with healthcare professionals across disciplines such as physicians, social workers, therapists, and home health providers to deliver seamless patient care.
Oversee documentation of medical records, electronic health records (EHR), and compliance with HIPAA regulations to maintain confidentiality and accuracy in patient information.
Educate patients and families on disease progression, symptom management, medication administration, and available support services to foster understanding and empowerment.
Facilitate communication between patients, families, and healthcare teams to ensure clarity of care plans and address any concerns promptly.

Qualifications

Valid nursing license with current registration; experience in hospice or palliative care preferred.
Critical care experience including ICU or emergency medicine settings highly desirable; familiarity with Level I Trauma Center or Level II Trauma Center experience is a plus.
Strong background in hospital medicine, medical-surgical units, or home health environments; outpatient or clinic experience is beneficial.
Knowledge of health regulation policies such as HIPAA along with proficiency in electronic health records (EHR) systems.
Demonstrated ability to perform comprehensive patient assessments utilizing vital signs measurement and diagnostic evaluation skills.
Experience managing patients with complex needs across diverse populations including geriatrics, pediatrics, behavioral health cases or infectious disease care.
Excellent client communication skills with the ability to collaborate effectively with multidisciplinary teams for optimal patient outcomes.
Familiarity with managed care principles, utilization management processes, discharge planning strategies, and post-acute care coordination.
