Senior Resource Center Case Manager

Join our dedicated team of case managers serving older adults throughout Jo Daviess and Stephenson Counties. As a Senior Resource Center Case Manager, you will play a vital role in helping seniors maintain their independence and quality of life.

Case Managers assess eligibility for programs and services by meeting with participants in their homes and conducting comprehensive needs assessments. This position also supports the Information and Assistance Department by helping clients navigate Medicare, Medicaid, benefit applications, and a variety of aging-related resources and services.

Qualifications

The ideal candidate will possess:

A bachelor's degree in a human services-related field, such as Social Work, Sociology, Psychology, Nursing, or a similar discipline
At least one year of experience in human services
Knowledge of Medicare, Medicaid, and other entitlement programs (preferred)
Strong attention to detail and the ability to adapt to changing situations
Excellent communication, problem-solving, and interpersonal skills
Strong organizational and time-management abilities
Proficiency in Microsoft Office and the ability to effectively use remote computer technology and equipment
