Job Title: Per Diem Registered Nurse (RN) / Licensed Practical Nurse (LPN)
Location: Access Care Partners
Job Type: Per Diem

Position Summary:
At Access Care Partners, we are seeking a compassionate and skilled Per Diem Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team. In this role, you will evaluate potential program participants in their homes, conduct initial visits, and provide ongoing re-evaluations under the supervision of an RN. You’ll play a key role in assessing consumers’ physical and cognitive conditions, ensuring that they receive the personalized care they need to maintain their well-being.

This role will predominantly be responsible for doing visits in the Franklin County area but may do visits as well in any of our other territories including Hampden and Hampshire counties, and surrounding areas.

As a Per Diem RN/LPN with Access Care Partners, you’ll have the opportunity to work on a flexible schedule, earning competitive compensation for your valuable contributions to our mission of providing top-tier care in the community.

Compensation:

$107 per Re-evaluation
$120 per Initial Visit
$30/hr for training hours

Essential Duties and Responsibilities:

Evaluate Consumers: Assess physical and cognitive conditions of program participants to determine their eligibility for personal care assistance services.
Conduct Assessments: Accurately determine the scope of personal care services needed for consumers, focusing on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Re-evaluations: Schedule and perform consumer re-evaluations in a timely manner according to state and federal regulations.
Collaborate with Team: Maintain regular communication with the RN Coordinator and Program Director to receive work assignments and submit completed assessments.
Consumer Support: Provide brief assessments of consumers' ability to manage the program and ensure they are receiving the best care possible.
Meetings & Trainings: Attend meetings as directed and stay current with training requirements to maintain high standards of care.

What We’re Looking For:

Registered Nurse (RN) or Licensed Practical Nurse (LPN) with an active Massachusetts license.
Valid driver’s license with a satisfactory driving record.
Ability to complete assessments and evaluations in a timely and professional manner.
Compassionate and patient-focused with a strong desire to make a difference in the community.
Availability for mutually agreed upon number of cases per month.

Certifications, Licenses, Registrations:

Current Massachusetts RN or LPN license.
Valid driver’s license and satisfactory driving record.
Satisfactory CORI, SORI, OIG, SAMS, and Exclusion List checks.
