Position Description
The basic function of a Direct Care Support Professional is responsible for the ongoing implementation care to come into the Rouses home to provide direct care and support as needed. Based on the Rouses' desires and personal interest caution and care is used to determine the most qualified competent person for this position. The Rouses have opened up their home for staff to assist them in their home with their daily needs. The following factors are brought into consideration when deterring if caregiver is a match within our environment:

Nothing is more important in this job than the relationships you develop with the Rouses and their family members. These relationships enable you to provide the best care possible and find true fulfillment in your job as care giver.
The Caregiver is responsible for promoting a harmonious relationship with co-workers and person served. The DCSP will incorporate activities in the home to ensure choice is incorporated throughout the residential setting. Staff will demonstrate a commitment to the principal of respecting and incorporating choice of person served during implementation of treatment plan.
The Direct Care Support Professional implements active treatment goals and services including serving as backup staffing for service of persons served in the absence of primary staff. The DCSP provides hands-on services innovation serves to persons served including but not limited to the following services: personal care, residential support, community guide, community networking, day supports, respite, and additional services identified as a need in the person served PCP.
It is the Caregiver’s responsibility to maintain communication with Latoya prior to changes to the approved schedule of the residence.
Other duties may be assigned as necessary.
Qualifications
- High School Diploma or General Equivalency Certificate. Associate Degree preferred
- Two years Group Home experience working with individuals with in health care setting is preferred.
- Documentation Ability to read and write detailed notes and instructions
- Possess a valid NC Driver’s License
- Computer Skills - Must have basic work processing (MS Word) and email usage skills.
- Operate a variety of office and other equipment (e.g. copier, telephone, vacuum, food preparation equipment, etc).
- Reasoning Ability - Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
- Available to work a flexible work schedule including weekends
Specific Duties
Administrative
- Maintain Confidentiality and HIPPA laws pertaining to any information concerning the Rouses and/or other individuals providing services for the Rouses
- Foster a homogeneous environment by establishing a cohesive relationship between co-workers, family and other supports within the home/community
- Maintain professional and ethical decorum at all times
- Follow home’s operational budget when handling money and receipts
- Ensure that dignity/respect and quality of life is maintained at the highest level for Rouse family
- Maintain an inventory and supply list to ensure each shift rotation has the necessary inventory and staples needed
- Provide transportation to Medical Appointments as requested
- Ensure teachable moments are incorporated throughout the each consumer treatment plan in the home
- Ensure Incidents, Medication Errors, Shift/Goal Notes and/or other documentation is completed and recorded daily
- Immediate report to Latoya Pettie if Incident and/or Medication Error and/or suspicions of abuse neglect or exploitation occurs in the home
- Receive, review, process, file and submit documentation pertaining to health and/or other scheduled appointments so each shift is aware of the date, place, time and reason appointment is scheduled.
Services
- Implementation consumer ISP in accordance to ICF/IDD regulatory guidelines
- Carry out each consumers’ treatment plan in a developmentally appropriate manner to meet the needs of each consumer served
- Serve as an advocate for each RGH consumer in the group home and community to ensure they exercise their rights to the fullest extent and provide assistance, as needed for rights they cannot fully execute independently
- Administer consumer medication as ordered by the physician and/or specialist
- Ensure consumers are free from abuse, neglect and/or exploitation
- Ensure implementation of consumer activity schedules remain current and reflect the interests/desires of the consumer
- Ensure that individuals are supervised at all times unless otherwise specified in their Individual Support Plan
- Provide ongoing communication with House Manager, Lead Staff, Nursing Staff and QMRP to maintain good overall health for consumers with notification of changes and/or improvements to treatments prescribed
- Provide ongoing communicate with co-workers, consumers, day program staff and community representatives in a professional and positive manner
- Assess needs; assist consumers with personal hygiene needs(male and/or female)
- Ensure doctor’s orders are followed as written and adaptive equipment is always in good repair
- Conduct and document as needed daily consumer house meetings to provide ongoing consumer training, notification of activities, and/or related opportunities available for them at RGH and in the community
- Ensure complete, accurate and quality documentation for assigned consumer
- Facilitate, transport and collaborate with House Manager to ensure health care professionals are provided information for the continuity of consumers’ well being
- Provide input in the evaluation of each assigned consumer for his/her Adaptive Behavior Inventory
- Ensure documentation and notification to House Manager, Lead Staff and QMRP of changes in consumers’ independent living strengths and needs
- Implementation consumers training objectives, health care procedures and service goals as needed
Residential Duties:
- Promote consumer choice in all aspects of residential treatment plan delivery
- Promote independence and provide flexible levels of supports as needed
- Ensure consumer can accurately and safely evacuate during House Fire and Tornado Drills
- Assist consumers in the development of natural support networks
- Assist consumers in development of community living skills
- Adhere, enforce and participate in the implementation and ongoing oversight of all RGH safety standards and regulations with the consumer in the home and in the community
- Perform emergency medical procedures as needed in accordance to RGH Emergency Management Procedures and OSHA regulations
- Ensure a clean, safe and orderly environment by performing general cleaning and housekeeping
- Ensure appliances, furniture and equipment is functional and operational to meet the consumers’ needs
- Assist with purchases of grocery for the home, inventory, and assist consumer with meal planning and preparation skills.
- Transport consumers, as needed
- Other duties as assigned
In-services and Training
- Attend, as assigned, regularly scheduled facility and agency in-services
- Assist in orientation, training and mentoring of other direct staff as requested
- Train on each consumer program objectives prior its initial implementation
- Maintain certifications for medication administration, NCI, first aid and CPR
- Maintain training to meet consumers’ individual needs including but not limited to medical procedures, therapies and/or behavioral support plans
- Participate as needed and document consumer training on consumer rights, house rules, other in-services to enrich the knowledge and awareness of the consumers’ served

Reasoning Ability

Physical Requirements
Must be able to operate a variety of equipment (e.g. wheel chairs, lifts, etc.). Must be able to exert up to seventy-five pounds of force occasionally and/or frequently to lift, carry, push, pull or otherwise move objects. Physical demands are in excess of sedentary work; position requires walking, standing, reaching, fingering, stooping, etc. Will lift no more than forty pounds independently. Must be able to lift and/or carry weights of up to one hundred and seventy-five pounds with assistance.
Other Qualifications
Must possess a valid NC Driver’s License; First Aid and CPR Training; Must be certified to administer medication within 90 days of employment
Schedule
Your schedule could vary day to day or week to week depending on the Rouses' needs. We attempt to maintain as much consistency as possible and take your availability into account when schedules change.
Our responsibility is to provide care services for the Rouses' 24/7.
Schedules will include during of the following times:
- Daytime
- Evening
- Overnight
- Weekend
- Holiday
Expectations:
- Transferring clients. Proper transferring/lifting techniques and on the use of patient lifts, such as the Hoyer Lift. Make sure caregivers have the proper training and supplies necessary when called on to transfer a client from one location to another.
- Bathing. Caregivers must know proper techniques and helpful tips for bathing clients who struggle with various physical challenges.
- Alzheimer’s and dementia. Caregivers need to know how to work with clients who suffer from these illnesses. Caregivers need to be aware of the symptoms and know how to provide consistent, reassuring, and compassionate care.
- Incontinence care. Prepare caregivers for what it may take to keep clients clean, dry and safe, and how to help clients keep their dignity despite potentially difficult or embarrassing situations.
- Safety. Caregivers and clients need to be safe in all situations. Safety training can cover on-the-job safety for caregivers and in-the-home safety for clients.
- Company policies, procedures and goals. The Rouses own their own Health Care Operation therefore their expectations within their home is the same as their business goals. They need to know how valuable they are to you and how the job you perform in the community reflects their business reputation as well. During your training and overtime policies for ensuring job duty expectations, rights, scheduling, reimbursements, communication, etc.
- Cooking and cleaning up meal related items. Caregivers are expected to cook meals balanced meals not offer fast foods and/or preprocessed food. Past situations have included difficulty in finding care givers that have cooking skills or don’t know how to cook their favorite foods. Caregivers may need basic cooking tips or simple menu plans to help them learn what the Rouses like to eat. Snacks and supplements are encouraged to maintain overall health and well being.
- Medication administration/order/refill.
- Conversation/Companionship. Providing conversation and encouraging the Rouses to share their thoughts and memories
- Exercise and physical activity. Ensure Mrs. Rouse stays as active as possible through healthy interaction and stimulation activities, including but not limited to daily physical fitness activities, outings and activities - Ongoing health interactions to promote both physical and mental well being
- Housekeeping. Performing housekeeping tasks such as dusting, vacuuming, making beds, changing linens, cleaning bathrooms and kitchens, washing and ironing laundry, etc.
- Errands
- Transportation
- Medical Appointment Scheduling/Accompaniment/Follow UP
- Daily Journal
Other responsibilities might be added to these lists, depending on their schedules and/or health status, but the most importantly this is an opportunity that is meant to be rewarding.
