What will you do? As a Mentor at Aspire Health Partners, your job purpose will be to provide a variety of essential functions to work collaboratively with program participants.
These functions include:

Providing direct support to program participants.

Working with multi-disciplinary teams to gather information and provide the best wraparound care possible for clients.

Documenting clearly and effectively in the Electronic Health Record.

Following all program specific protocols in relation to specified role, maintaining fidelity to program model.

Maintaining all required credentials necessary to perform job functions.
Qualifications

High School Diploma
-
Specialized certification as a Peer Recovery Specialist or Parent Coach may also be required, based on specific program.
