RESIDENT CARE COORDINATOR

JOB DESCRIPTION

TITLE: Resident Care Coordinator

DEPARTMENT: Resident Care Services

EDUCATION/TRAINING

EXPERIENCE: Administrator

Must hold a current and unencumbered LPN license in the appropriate state of hold a current Nurses Aid Training Certificate in the appropriate state. LPN preferred.

JOB REQUIRMENTS:

General Health Requirements to include the two step TB testing.

RESPONSIBLITIES: The Resident Care Coordinator will:

Perform all duties as required if the SIC/Medication Aide.
Assure that all admission requirements and paperwork are satisfied.
Assure that all new orders are transcribed correctly.
Be available to answer questions of the families and/or residents.
Monitor MAR’s and Personal Flow Sheets for appropriate documentation.
Order Medical Supplies as needed.
Orient new staff (Nursing Aides and SIC/Medication Aide).
Serve as liaison between Resident Care Services AND Administrator
Assure that monthly Vital Signs and Weights are completed by the 10th of each month.
Assure that Medication is ordered in a timely manner.
Assign shift responsibilities, i.e., cleaning wheelchairs, vital signs, showers, etc.
Follow-up with physicians as needed.
Make home health referrals and doctor’s appointments.
Monitor resident records for nurses’ notes.
Review incident log on a daily basis and follow up as necessary.
Make shift assignments as necessary.
Assure disciplinary producers are enforced according to policy.
Communicate with staff regarding new orders, new residents, or status changes in residents.
Conduct Residents Care meeting on a bi-monthly basis in conjunction with the Administrator.
Assure the residents/staff TB Est, chest X-rays as appropriate, and annual TB screening are done.
Ensure that Care Plans are done appropriately and in a timely manner.
Assure that required forms are current and orders are clarified as indicated.
Ensure optimal clinical integrity and overall resident care.
For LPN’s only: The LPN is NOT to use his or her professional notation (LPN after signature) when signing any documents in the Adult Care Home. The LPN is functioning as the supervisor consistent with the Rules for the Licensing of Adult Care Homes. In nursing care is provided by the LPN beyond the scope of the Adult Care home this care must be done in compliance with the nursing law, i.e. registered nurse must be continuously available to him/her, on site when necessary.
Other duties as assigned.
