Full-Time Registered Nurse (RN)- Clinical Lead

This position may also be per-visit or hourly rate

Location: Las Vegas and surrounding areas- including Henderson and North Las Vegas

As our agency continues to expand, this position is designed to transition into a Clinical Lead RN role for the right individual.

Position Summary

We are seeking an experienced, compassionate, and highly motivated Registered Nurse to join our growing clinical team.

This position provides skilled nursing care to patients in their homes while serving as a key contributor to our culture of clinical excellence. The ideal candidate enjoys autonomy, takes pride in delivering exceptional care, and wants to be part of an organization that is raising the standard for home health.

For nurses interested in leadership, this position offers a defined pathway toward becoming our future Clinical Lead RN.

Responsibilities

Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and routine skilled nursing visits.
Perform comprehensive patient assessments.
Develop and update individualized plans of care.
Educate patients and caregivers.
Coordinate care with physicians and interdisciplinary team members.
Accurately complete OASIS documentation.
Identify changes in patient condition and communicate appropriately.
Maintain compliance with Medicare Conditions of Participation and agency policies.
Deliver exceptional patient-centered care while maintaining excellent documentation standards.

Qualifications

Current Nevada RN license.
Minimum one year of RN experience (home health experience preferred).
Strong clinical assessment and critical thinking skills.
Excellent communication and organizational abilities.
OASIS experience preferred but not required.
Current BLS certification.
Reliable transportation and valid driver's license.

View themselves as an advocate for their patients, not just someone completing scheduled visits.
Follow through. If they identify a concern, they don't assume someone else will handle it-they take ownership until it's addressed.
Communicate proactively with physicians, families, therapists, and office staff to ensure exceptional continuity of care.
Treat every patient's home with respect and every patient with dignity, compassion, and kindness.
Strive to continually improve their clinical knowledge while maintaining the highest standards of documentation and patient care.
