Job Title: Housing Supports / HRA CARES Case Manager

Key Responsibilities
- Community-based position requiring a minimum of 50% of the workday to be spent in the community.
- Coordinate housing, healthcare, financial, and community supports for individuals experiencing homelessness or housing instability.
- Manage all programmatic requirements for participants receiving Housing Supports and HRA CARES services.
- Advocate for clients to obtain and maintain safe, stable, long-term housing.
- Complete housing assessments, develop individualized service plans, and maintain accurate, timely, and compliant documentation.
- Assist participants in developing independent living skills that promote long-term housing stability.
- Apply trauma-informed, person-centered, and culturally responsive practices that respect each individual’s circumstances and promote equitable access to services.
- Build and maintain partnerships with landlords, housing providers, healthcare organizations, county agencies, and community resources.
- Coordinate services with internal departments to ensure continuity of care and comprehensive client support.
- Monitor participant progress, identify barriers to housing stability, and implement effective interventions.
- Participate in ongoing training, professional development, and quality improvement initiatives.
- Maintain compliance with all Housing Supports, HRA CARES, Medicaid, DHS, and organizational policies and procedures.
Required Qualifications & Skills

Education & Experience
- Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, Public Administration, or a related field preferred.
- Relevant experience working with individuals experiencing homelessness, mental illness, substance use disorders, or other barriers to housing is preferred.
Knowledge & Skills
- Knowledge of housing stabilization strategies, community resources, and social service systems.
- Understanding of physical health, mental health, trauma-informed care, human development, and cultural responsiveness.
- Strong critical thinking, problem-solving, and independent decision-making skills.
- Excellent written and verbal communication skills.
- Ability to build positive relationships with clients, landlords, healthcare providers, and community partners.
- Strong organizational skills with the ability to manage multiple priorities and maintain accurate documentation.
- Ability to maintain confidentiality and handle sensitive information in accordance with HIPAA, DHS, and organizational policies.
- Knowledge of Fargo-Moorhead housing resources and community support systems is preferred.
- Commitment to providing equitable, person-centered services that empower clients toward independence.
Requirements
- Must possess a valid driver’s license, reliable personal vehicle, and current automobile insurance.
- Must be willing and able to travel throughout the service area on a daily basis.
- Must successfully pass a Minnesota Department of Human Services (DHS) background study prior to employment and maintain eligibility throughout employment.
- Ability to lift up to 25 pounds and perform community-based work in a variety of settings.
- Ability to use computers, electronic health records, and Microsoft Office applications.
