Job Title: Physician Assistant (PA-C) | Primary Care | Community-Based Medicine

Location: Southern Minnesota | Owatonna, Rochester, Albert Lea, and Surrounding Communities

Compensation: $120,000 - $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Bring Primary Care Directly to the Patients Who Need It Most.

You'll round throughout assisted living, memory care, independent living, and skilled nursing communities across Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities.

Instead of spending every day inside the same clinic, you'll bring primary care directly to your patients.

You'll diagnose and treat acute illnesses, manage chronic medical conditions, perform routine follow-up visits, coordinate care after hospitalizations, and collaborate with interdisciplinary teams to keep patients healthier and living safely within their communities.

You'll also have something increasingly difficult to find in traditional medicine: continuity.

You'll know your patients.

You'll understand their histories.

You'll recognize when something has changed.

And you'll have the clinical autonomy to help determine what happens next.

Your expertise won't simply improve today's visit.

It can help shape a patient's health for months and years to come.

What You'll Be Doing

Provide comprehensive primary care services throughout senior living communities.
Evaluate, diagnose, and treat acute and chronic medical conditions.
Perform routine follow-up visits, annual assessments, and preventative care.
Develop individualized treatment plans focused on long-term health and quality of life.
Manage chronic diseases and medically complex patients.
Identify changes in condition and intervene proactively.
Coordinate care following hospitalizations, rehabilitation stays, and specialist consultations.
Partner closely with physicians, nurses, therapists, pharmacists, social workers, caregivers, and community leadership.
Educate patients and families while helping them make informed healthcare decisions.
Maintain accurate and timely clinical documentation.
Support quality improvement initiatives focused on reducing avoidable hospitalizations and improving patient outcomes.

What We're Looking For

Required Qualifications

Graduate of an accredited Physician Assistant program.
Active Minnesota PA-C license in good standing.
Excellent assessment, diagnostic, and clinical decision-making skills.
Ability to practice autonomously while collaborating within a physician-led team.
Strong communication and care coordination skills.
Passion for caring for older adults.
Reliable transportation and willingness to travel throughout Southern Minnesota.

Preferred Qualifications

Experience serving seniors or medically complex patients.
Chronic disease management expertise.
Community-based healthcare experience.
Assisted living, skilled nursing, or long-term care experience.
Wound Care Certification (WCC) is a plus.
