CASE MANAGER

SUICIDE PREVENTION (SSG Fox SPGP)

Position Title: Case Manager Suicide Prevention (SSG Fox SPGP) Department/Program: Veterans Services of the Carolinas (“VSC”) Reports To: Program Director Suicide Prevention (SSG Fox SPGP)

Location: Fayetteville, North Carolina

FLSA Status: Exempt

Work Schedule: Full-time position requiring flexibility, including evenings, and weekends.

Compensation: Annual salary commensurate with education and experience.

POSITION SUMMARY

The Case Manager – Suicide Prevention provides case management, care coordination, and participant support for veterans and individuals at risk of suicide or experiencing behavioral health challenges. This position develops individualized service plans, coordinates behavioral health and community resources, supports crisis intervention efforts, and works collaboratively with healthcare providers, the U.S. Department of Veterans Affairs, and community partners to promote participant safety, recovery, and continuity of care.

ESSENTIAL DUTIES & RESPONSIBILITIES

Case Management & Participant Support

 Provide comprehensive case management and care coordination for veterans and individuals experiencing behavioral health challenges, suicidal ideation, trauma, substance use disorders, social isolation, or related concerns.

 Complete participant assessments, develop individualized service plans, establish goals, and monitor participant progress.

 Assist participants in accessing behavioral health treatment, healthcare services, crisis intervention, housing resources, benefits, employment services, transportation, and community supports.

 Promote participant engagement, recovery, and long-term stabilization through coordinated service planning.

Suicide Prevention & Crisis Coordination

 Identify suicide risk factors and behavioral health concerns and respond in accordance with program protocols.

 Support safety planning, crisis intervention, and participant stabilization using trauma-informed and recovery-oriented practices.

 Coordinate with behavioral health providers, crisis response agencies, VA personnel, and community partners to ensure continuity of care.

 Conduct participant follow-up to encourage engagement and reduce barriers to services.

Resource Coordination & Documentation

 Coordinate referrals with healthcare providers, behavioral health agencies, VA systems, housing organizations, and community resources.

 Maintain timely and accurate documentation of participant interactions, referrals, service plans, and outcomes.

 Utilize HMIS, data portal, and other required reporting systems.

 Ensure compliance with grant requirements, confidentiality standards, and organizational policies.

Team Collaboration & Ministry Support

 Collaborate with peer support specialists, outreach staff, healthcare providers, churches, volunteers, VA personnel, and community partners.

 Participate in interdisciplinary meetings, training, ministry initiatives, and organizational activities.

 Promote ABCCM's mission and values in all participant and community interactions.

 Perform other duties as assigned.

QUALIFICATIONS

 Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Behavioral Health, or a related field preferred; equivalent experience may be considered.

 Experience in case management, behavioral health, suicide prevention, crisis intervention, veteran services, homelessness services, or related human services preferred.

 Clinical licensure or licensure eligibility preferred, if required by program needs.

 Experience working with vulnerable populations and interdisciplinary service systems preferred.

 Strong computer, documentation, and reporting skills required.

 Valid North Carolina driver's license and reliable transportation required.

PHYSICAL REQUIREMENTS / WORKING CONDITIONS

Work is performed in office, community, healthcare, residential, and outreach environments throughout Western North Carolina. Responsibilities require regular travel, prolonged sitting, standing, walking, and operation of a motor vehicle, use of standard office equipment, and occasional lifting and carrying of materials weighing up to 20 pounds. The position regularly encounters individuals experiencing behavioral health conditions, trauma, suicidal ideation, substance use disorders, and other complex service needs.

MENTAL DEMANDS

Must be able to manage multiple participant needs, crisis situations, documentation requirements, referrals, and interdisciplinary communications simultaneously. Requires strong communication, critical thinking, emotional resilience, conflict resolution, and sound judgment while serving individuals experiencing behavioral health crises and suicide risk.
