Full Job Description

We provide intermittent home health care, palliative care, hospice care, skilled private care services.

We are looking for a compassionate and experienced Medical Social Worker to join our interdisciplinary team. As a Medical Social Worker, you will assist patients and families to cope with psychosocial challenges to achieving their health care goals. You will utilize a holistic approach to assess patient and family/caregivers needs relative to coordinating appropriate care. You will develop, implement, and update interdisciplinary treatment plans, consult with and educate patient and family and facilitate referrals to other healthcare professionals and programs. You will participate in team meetings and required in-service training's. Most of this work will be within the hospice / palliative care department with some home health related assignments possible.

The ideal candidate will have:

· Minimum of 1 year of Medical Social Work experience in hospice

· Present a compassionate and caring demeanor with the ability to build a rapport with clients

· Possess excellent communication and customer service skills, be well organized, and be comfortable with electronic documentation

· Homecare Homebase experience a plus.

· Prior hospice/palliative care experience a plus

Hospice / palliative Care: 1 year (Preferred)
Medical Social Work: 1 year (Required)

License:

Fully licensed prefer, but limited license are welcome to apply (supervision is available).

Responsibilities

Provide emotional and psychological support to patients and their families in hospice care settings.
Conduct patient assessments and develop individualized care plans based on needs.
Maintain accurate medical documentation and adhere to HIPAA regulations.
Collaborate with healthcare teams to ensure comprehensive patient care.
Facilitate communication between patients, families, and healthcare providers to enhance service delivery.
Engage in crisis intervention and provide counseling as needed.

Pay: $63,000.00 - $95,000.00 per year
