MDS Coordinator

Job Summary:

Axiom Healthcare of Flora is seeking a highly skilled and dedicated DNP MDS Coordinator to join our team. As a vital member of our healthcare community, you will play a pivotal role in ensuring the smooth and efficient coordination of patient care services. Responsibilities:

Coordinate the development and implementation of patient care plans in collaboration with the interdisciplinary team
Conduct thorough assessments of patients' needs and preferences to ensure personalized care
Implement and maintain accurate and up-to-date patient care records and data systems
Collaborate with healthcare providers and staff to ensure comprehensive and coordinated care plans
Provide education and training to patients, families, and caregivers on patient care plans and procedures
Participate in quality improvement initiatives to ensure the highest level of patient care and satisfaction

Requirements:

Minimum 2 years of experience in a similar role or setting
Strong clinical skills and knowledge of healthcare regulations and standards
Excellent communication, organizational, and leadership skills
Ability to work collaboratively as part of a multidisciplinary team
Strong attention to detail and ability to maintain accurate records and data
