Full Job Description

Job Summary
We are seeking a dedicated and highly skilled Registered Nurses (RN's) to coordinate and deliver comprehensive patient care within a home health setting. As a vital member of our team, you will oversee patient assessments, develop individualized care plans, and facilitate seamless communication among healthcare providers, patients, and families.

Duties

Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.
Develop and implement personalized care plans that align with clinical guidelines and patient goals, incorporating discharge planning and post-acute care strategies.
Coordinate with multidisciplinary teams to facilitate diagnostic evaluations, medication management, and therapeutic interventions in the home environment.
Monitor patient progress through regular follow-ups, adjusting care plans as necessary to optimize recovery and manage chronic conditions.
Manage documentation of medical records while ensuring compliance with HIPAA regulations and maintaining accurate case management records.
Provide education on disease management, medication adherence, and health maintenance tailored to diverse populations such as geriatrics, pediatrics, or behavioral health patients.
Facilitate communication with primary care providers and specialists to ensure continuity of care across inpatient, outpatient, and clinic settings.
Engage in utilization management by evaluating the appropriateness of services and advocating for necessary resources within managed care frameworks.

Requirements

Valid RN license with current registration in the state of practice.
Experience working with diverse patient populations including geriatrics, pediatrics (PICU), behavioral health patients, and those requiring post-acute or outpatient care beneficial but not required.
Familiarity with medical terminology, medical records management, discharge planning procedures, and HIPAA compliance standards.
Excellent patient assessment skills
Strong organizational skills with attention to detail in documentation and medical recordkeeping.
Prior experience in Home Health Care is preferred but not required, will train qualified applicants.

We are looking for RN Care Managers to see clients in Northern Kent (196 north to Cedar springs) and Montcalm County. Your patients would be centered around your home location. Position is paid by the type of patient visit (not hourly) plus mileage reimbursement. Per week visit range can be up to 25 patient visits per week for full time and as little as you would like for part time. We do lots of wound care and home I.V. Infusions. Home Health Care Case Managers coordinate patient care with physicians and other disciplines involved with the patients care. Variety in day-to-day care makes this position unique. Paid Meeting reimbursement and CEU's also offered.

Pay: $65.00 - $80.00 per hour
