Full Job Description

Part-time, prn, and Full-time day shift positions available.

Come make a difference in the lives of our elders, and provide quality, person centered care for residents of a long term care facility. Job Duties:

Report all unusual conditions, reactions or complaints of residents to nurse using the “Stop and Watch” tool unless it’s an emergency situation, then notify nurse immediately.
Assist residents with personal hygiene, grooming, oral care, nail care (except diabetic resident or those receiving blood thinners), bathing, dining and any other physical tasks assigned within scope of training.
Provide resident privacy by knocking on doors or ringing doorbells and greeting residents prior to entering room and pulling curtain or closing door when rendering personal care.
Turn and reposition appropriate residents every two hours, or as per resident need, using pillows for positioning and comfort; provide frequent weights shifts if up in chair/wheelchair.
Provide comfort measures, assist residents with toileting needs per care plan/toileting schedule; keep personal articles within reach of resident.
Prepare residents for meals by assisting residents with oral hygiene and hand washing before meals.
Supervise meals to be sure all residents’ dining needs are met; provide assistance and encouragement with eating as residents’ condition indicates.
Apply alarms, therapeutic devices and TED hose per resident’s care plan to maintain positioning, promote mobility and safety; ensure that devices are functioning properly.
Check activity calendar daily and encourage residents to participate in scheduled and spontaneous activities; escort/transport residents to and from activity programs per residents’ interests.
Spend free time with residents providing conversation and companionship, assessing safety and addressing needs/concerns.
Promptly answer call lights. Anticipate residents’ needs, check on residents regularly and demonstrate a cooperative attitude when assisting residents and when communicating with family members.
Transfer and ambulate residents in a safe manner using proper mechanics, appropriate assistive devices and a gait belt in accordance with the facility’s lifting policy and as indicated by care plan.
Accept assignment per schedule, perform physical rounds with previous/oncoming shift and discuss specific instructions for care with the nurse.
Assist with orientation of new staff as needed; provide a welcoming and friendly atmosphere to new employees of all departments.
Ensure continuation of adequate supply of individual resident care times, i.e. gloves, dental care items, barrier cream, undergarment, etc.
Make sure residents’ call cords are within reach at all times when in room.
Measure and record height, weight, vital signs and O2 sats on admission and as instructed by nurses.
Measure and record fluid and food intake as instructed by the nurse.
Keep residents’ rooms, neighborhoods and nurses’ stations neat and clean; make occupied and unoccupied beds and change linens per schedule and as needed.
Document resident care daily in the computer system. If care is refused, it should be reported to the nurse immediately and should be documented as a refusal. Follow care plan when administering care.
Offer residents and/or assist residents with fluids frequently. Keep water within residents reach when in their room.
Ensure residents’ O2 is in place and tanks are filled as directed by the nurse; report any shortness of breath, change in skin color and/or difficulty breathing to the nurse immediately.
Report clothing needs and missing/lost possessions to the Social Service department.
Provide preventative skin care, and report any skin concerns to the treatment nurse.
Ensure that any chemicals used are properly labeled and locked up when not in use.
Keep equipment clean and in working order. Report equipment malfunctions or needs to the nurse.
Assist with evacuation of residents as directed according to the facility Emergency Operations Plan.

Pay: $17.60 - $22.88 per hour
