Job Description :

Intensive Case Management Supervisor (ICMS)

I. General

The Intensive Case Management Supervisor oversees the day-to-day operations of the Intensive Case Management (ICM) program. ICM is a community-based service that provides mental health support, care coordination, linkage to needed services, and interventions designed to promote recovery, stability, and successful community living.

The ICM Supervisor is responsible for ensuring that ICM services are provided in accordance with DBHDD requirements, Medicaid standards, CFI policies and procedures, and applicable regulatory and funding requirements. The Supervisor provides direct supervision and oversight of ICM Case Managers; monitors caseloads, service delivery, documentation, and program performance; and ensures individuals receive high-quality, person-centered services consistent with their assessed needs and Individualized Recovery Plans (IRPs).

The ICM Supervisor periodically accompanies ICM Case Managers on community visits to monitor service quality, support staff safety, and provide coaching and supervision as needed. This position reports to the Case Management Program Director.

II. Major Duties and Responsibilities

· Monitors the caseloads of the team and reaches out to referral sources to maintain at budgeted capacity

· Provides ongoing supervision, coaching, performance monitoring, and support to ICM case managers to ensure effective service delivery and compliance with program requirements.

· Conducts the initial intake with individuals referred, including review of referral information, completion of required intake documentation, assessment of immediate needs, and orientation to ICM services. Upon completion of the intake and determination that the individual meets program admission criteria, assigns the individual to ICM case manager.

· Reviews and approves IRPs and SRSs

· Accompanies ICM case managers on client visits as needed to ensure safety of ICM case manager and quality of service.

· Reviews ICM documentation in the EHR on a minimum of weekly basis to ensure services and documentation meet DBHDD, Medicaid, and CFI requirements for medical necessity, service definition, billing, timeliness, and documentation quality. Provides timely feedback and corrective guidance to staff when documentation does not meet required standards.

· Monitors ICM documentation to ensure progress notes clearly identify the intervention provided, match the identified service definition, the individual’s response, progress toward IRP goals and objectives, and the plan for continued services, as applicable to the service provided.

· Assists with training and mentoring newly hired staff.

· Conduct and documents weekly ICM Team Meeting. Ensures each individual served is discussed at least monthly and that required Treatment Team meeting Logs are completed and maintained.

· Assists ICM case managers with referrals to needed client services such as medical, mental health, wellness, social, educational, vocational/employment, housing, financial, co-occurring needs, natural support, and community integration.

· Monitors individuals served housing stability and housing-related needs and ensures ICM case managers assist them with obtaining and maintaining housing of their choice as appropriate to their needs and goals.

· Oversees transition and discharge activities in accordance with DBHDD requirements and CFI policy.

· Monitors coordination with CoC, housing providers, medical and behavioral health providers, residential programs, employment services, crisis services and other providers to promote continuity of care and prevent duplication of services.

· Monitors completion of Client Satisfaction Surveys.

· Attends monthly meetings, i.e. Nick and Fulton County.

· Attends DBHDD – Region 3 ICM Program meetings

· Builds and maintains collaborative partnerships with community organizations, service providers and public services to ensure clients have access to a comprehensive array of supportive services.

· Communicates regularly with direct supervisor regarding program needs, challenges and other critical program information.

Maintains open lines of communication with ICM case managers to assist with awareness of client achievements and challenges.

Quality and Quantity

· Maintains individual case manager caseloads at CFI’s target of 1:20 and ensures caseloads do not exceed applicable DBHDD staffing ratios.

· Ensures CFI’s 72-hour contact standard and all DBHDD referral-to-engagement requirements are met.

· Ensure that client contact is consistent with the DBHDD Service Definition and Guidelines. Monitors ICM service frequency, intensity, modality, and location to ensure compliance with current DBHDD ICM service-definition requirements and individual’s IRP.

· Monitors job performance to ensure that agency goals and objectives are achieved.

· Completes the monthly Think HIE (ICM) Supplemental Measures report demonstrating program impact and outcomes.

· Completes monthly ICM Program Narrative submitted to CFI’s CFO.

· Reviews and completes probationary and annual performance reviews.

III. Competencies, Knowledge, Skills, Abilities

· Demonstrate the ability to lead, inspire, motivate and build relationships with ICM clients, their families and their support systems

· Establish and maintain necessary professional relationships with other community resources, agencies and service providers.

· Excellent written, verbal and interpersonal communication skills

· Ability to work independently in the field with limited supervision, making sound, independent judgement and creative problem solving.

IV. Qualifications:

· Must be a licensed mental health professional with preferably a minimum of two (2) years of experience in a supervisory role.

· Must be able to understand and perform continuous quality improvement practices.

· Use of a personal vehicle; a current Georgia driver’s license, and be insurable in the state of Georgia.

V. Physical Requirements

Must be able to lift 25 – 50lbs, retrieving and placing materials on high shelves and on

Floor-level shelves; standing, walking; carrying; grasping; hearing; seeing; sitting;

Kneeling; climbing; balancing; stooping; crouching and reaching.
