Full Job Description

Job Overview

A Medicare-certified home health agency is seeking a motivated and self-directed Registered Nurse (RN) to join its team as an RN Case Manager focused on admissions and growth for the Gainesville region. This is a flexible, part-time position combining field and office work, ideal for a candidate who enjoys building relationships with referral sources and guiding new patients through the start of home health services. The Gainesville region covers a 30-mile radius around our Gainesville office.

The ideal candidate is a strong communicator, comfortable working independently in the field, and skilled at managing incoming referrals while identifying opportunities to grow the agency's presence in the Gainesville area. Previous home health, admissions, or case management experience is preferred.

Key Responsibilities

Conduct in-home admission visits for new patients referred to home health services within the Gainesville region (a 30-mile radius of the Gainesville office).
Complete comprehensive admission assessments (OASIS), establish the initial plan of care, and initiate services in accordance with physician orders.
Manage incoming referrals for the Gainesville region, ensuring timely follow-up and smooth transition of patients into care.
Build and maintain relationships with physicians, hospitals, facilities, and other referral sources to support growth in the region.
Identify and pursue opportunities to expand patient census and community presence within the Gainesville service area.
Communicate patient needs, plan of care details, and admission outcomes to office staff, field clinicians, and the care team.
Coordinate with the scheduling and case management team to ensure a smooth handoff after admission.
Educate patients and caregivers on the home health process, plan of care, and expectations for services.
Maintain accurate, timely, and complete clinical documentation within the electronic medical record.
Communicate changes in a patient's condition, physician orders, or plan of care to the appropriate members of the care team.
Ensure services are initiated in accordance with physician orders, agency policies, and applicable home health regulations.
Respond professionally to calls and messages from patients, families, referral sources, and physicians.
Maintain patient confidentiality and comply with HIPAA requirements.
Complete additional clinical and administrative duties related to admissions and growth as assigned.

Qualifications

Current and unrestricted Florida RN license.
Current Basic Life Support (BLS) or CPR certification.
Previous home health experience preferred; admissions, OASIS, or case management experience a plus.
Familiarity with home health regulations, OASIS, and documentation requirements preferred.
Experience with Axxess Home Health or another electronic medical record system is a plus.
Reliable transportation and willingness to travel within the Gainesville region (30-mile radius of the office) for in-home visits.
Strong computer skills, including proficiency with email and Microsoft Office.
Excellent written and verbal communication and relationship-building skills.
Self-motivated with strong organizational and time-management abilities.
Ability to work independently and manage a flexible schedule.
Ability to communicate professionally with patients, families, physicians, referral sources, and coworkers.
High level of accuracy and attention to detail.
