Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with extensive experience across various clinical settings. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. Your expertise will contribute to creating a safe, respectful, and healing environment for diverse patient populations, including those with complex medical needs. This position offers an opportunity to make a meaningful difference in the lives of patients while working within a dynamic healthcare facility committed to excellence.

Responsibilities

Provide direct patient care by monitoring vital signs, administering medications, and performing routine assessments in accordance with physician orders and established protocols.
Assist with medical procedures such as catheterization, IV infusion, wound care, and specimen collection while maintaining sterile technique and infection control standards.
Document patient information accurately using electronic health records (EHR/EHR systems) and ensure compliance with HIPAA regulations to protect patient confidentiality.
Support discharge planning and coordinate care with multidisciplinary teams to facilitate smooth transitions for patients across settings such as hospitals, skilled nursing facilities, or home care.
Educate patients and their families on health management strategies, medication administration, and post-discharge instructions to promote ongoing wellness.
Respond promptly to emergency situations including first aid, airway management, or acute pain episodes while adhering to clinical confidentiality policies.
Collaborate effectively with physicians, therapists, case managers, and caregivers to develop individualized care plans tailored to each patient's needs—especially those requiring specialized attention such as dementia care or Alzheimer's support.

Experience

Valid LPN license in good standing; additional certifications such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) preferred.
Demonstrated experience working in diverse clinical environments including long-term care facilities, assisted living communities, hospitals (including ICU or trauma centers), or outpatient clinics.
Proficiency with EMR/EHR systems such as Cerner, Epic, Athenahealth, or eClinicalWorks for accurate documentation and communication.
Extensive background in managing complex patient cases involving geriatrics, pediatrics, behavioral health, or infectious disease care.
Hands-on experience performing procedures like venipuncture, tube feeding management, sterile dressing changes, and medication administration—including injections and IV insertions.
Knowledge of long-term care regulations and familiarity with ICD coding (ICD-9/ICD-10) for accurate documentation of diagnoses.
Strong understanding of physiology principles related to patient assessment and clinical evaluation across acute care settings such as emergency rooms or surgical units.
