Full Job Description

Designated Coordinator (DC) - Charm Care Services

Charm Care Services is seeking a qualified and dedicated Designated Coordinator (DC) to oversee the coordination, monitoring, and evaluation of person-centered services and support plans for individuals receiving support under Minnesota DHS 245D licensing. This role is essential to ensuring individuals receive high-quality, compliant, and person-centered care that promotes independence, dignity, and well-being.

Overview
As the Designated Coordinator, you will serve as the primary point of coordination between our individuals, their support teams, county case managers, and direct support staff. You will ensure each person's Support Plan Addendum is implemented correctly, monitor service delivery, and ensure full staff and operational compliance with Minnesota 245D licensing standards. You'll also provide direct oversight to Lead DSPs and DSPs, helping build a strong, well-supported direct care team. This is a hands-on leadership role for someone who is organized, compassionate, and committed to person-centered practices.

Responsibilities

Create, monitor, and evaluate individual Support Plans and Support Plan Addendums.
Provide supervision, support, and evaluation of direct care service delivery.
Directly supervise Lead DSPs, delegating and overseeing day-to-day coordination tasks while maintaining accountability for outcomes.
Support and Mentor DSPs, will be responsible for coaching and onboarding DSP staff.
Ensure ongoing compliance with Minnesota 245D licensing standards and agency policy.
Serve as the primary communication link between individuals supported, families/legal representatives, county case managers, and direct support staff.
Manage incident reports and Individual Abuse Prevention Plans (IAPP), ensuring timely and accurate documentation.
Ensure the rights, dignity, safety, privacy, and choice of each individual are upheld at all times.
Conduct regular satisfaction evaluations with individuals served, legal representatives, and case managers.
Support and guide direct support staff, including Lead DSPs, to ensure consistent, high-quality care.
Maintain accurate, up-to-date documentation and records required for licensing and service delivery.
Assist with staff training, orientation, and ongoing compliance support related to 245D standards.
Respond to referral, intake, and service coordination needs as they arise.

Required Qualifications
Candidates must meet Minnesota's 245D Designated Coordinator requirements by satisfying a minimum of one of the following pathways:

Option 1 – Bachelor's Degree

Bachelor's degree in a field related to human services, and
1 year of full-time direct care experience with individuals with disabilities or adults age 65+

Option 2 – Associate Degree

Associate degree in a field related to human services, and
2 years of full-time direct care experience with individuals with disabilities or adults age 65+

Option 3 – Human Services Diploma

Diploma in a human services-related field from an accredited postsecondary institution, and
3 years of full-time direct care experience with individuals with disabilities or adults age 65+

Option 4 – Education & Training

At least 50 hours of education/training related to human services and disabilities, and
4 years of full-time direct care experience under the supervision of a qualified staff person

Additional requirements:

Ability to pass a Minnesota DHS background study
Ability to read, write, and communicate effectively in English
Valid driver's license and acceptable driving record

Preferred Qualifications

Prior experience working in a Minnesota DHS 245D-licensed program
Strong working knowledge of 245D requirements and person-centered service delivery
Experience communicating with county case managers, social service agencies, or healthcare providers
Prior experience supervising, coordinating, or mentoring staff, such as serving as a Lead DSP or DSP Mentor
Prior supervisory or coordination experience in a human services setting
Strong organizational, documentation, and follow-up skills
Ability to work independently and manage multiple priorities

Knowledge, Skills & Abilities

Solid understanding of 245D service delivery and compliance expectations
Strong leadership, communication, and interpersonal skills
High attention to detail, particularly in documentation and reporting
Sound judgment and problem-solving ability within scope of role
Commitment to person-centered practices and individual rights
