Full Job Description

Overview:

The hours for this position are 8am to 4pm M-F some evenings.

The Structured Day Program Coordinator is responsible for the care of all assigned participants enrolled in the Department Of Health (DOH) Traumatic Brain Injury (TBI) and Nursing Home Transition and Diversion (NHTD) waiver program. The Structured Day Program Coordinator is responsible to ensure a safe environment, implement individualized goal plans, advocate for the needs of the participants and support individuals in active community involvement. Furthermore, this position is responsible to assist participants in various interest-based and/or vocationally driven activities.

The Structured Day Program is moving towards growth and expansion to better serve participants. The hours for this position are 8am to 4pm M-F. This is an exciting new opportunity for someone with passion and drive, who is looking for growth within the company. Responsibilities:

Essential Functions:

Complete daily progress session note templates for all encounters with the participants.
Complete Individual Service Reports (ISR) as required to document goals, progress, and recommendations.
Attend and participate in scheduled Team Meetings for participants served to review the current Service Plan twice annually as scheduled by the Service Coordinator. The SDP trainer is expected to notify their supervisor in the event they are unable to attend a scheduled meeting, so that coverage by the supervisor can be arranged.
Adhere to documented goals for each participant and assist in various activities, pertaining to such goals.
Meet with supervisor on a routine basis (as determined by supervisor) for support and feedback.
Mediate and report emergency situations pertaining to the participant according to established policies and procedure and ensure safeguards of such situations in a timely fashion.
Ensure that each participant is treated with dignity and respect in accordance with the Department of Health and New York State regulations and Agency policy.
Report possible violations of participants’ rights promptly to supervisor, so that they may complete and submit incident reports in accordance with New York State regulations and Agency policy.
Maintain the confidentiality of any information relating to the program participants and staff of the program.
Ensure that the participant’s needs are met and intercede on their behalf as necessary.
Attend and complete trainings in accordance with New York State regulations and Agency policy.
Perform additional responsibilities as requested and/or necessary to best serve program participants. .
Mediate emergency situations to the program according to established policies and procedures and ensure communication of such situations in a timely fashion.

Additional Responsibilities:

Demonstrate professional conduct at all times.
Accurately and positively communicate Agency philosophy and administrative.
Receive and communicate effectively on feedback from management, peers, and participants/advocates in a cooperative manner.
Perform other duties as assigned.

Qualifications:

Bachelors degree in Education/ Health related field.
Preferred Masters degree in Therapeutic Recreation
Preferred Masters degree in education or Occupational Therapy or Psychology
Minimum of one year experience. Must include paid experience providing supports and services to people with varied abilities/Traumatic Brain Injury
Availability to work evening and or weekends.
NYS valid driver's license

Job Type: Full-time

Pay: $25.00 - $30.00 per hour

Expected hours: 40 per week
