Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This role involves performing patient assessments, administering medications and treatments, and supporting overall patient well-being. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering respectful, patient-centered care. This position offers an opportunity to make a meaningful difference in the lives of diverse patient populations while working within a collaborative healthcare environment.

Responsibilities

Conduct comprehensive patient assessments, including vital signs and physical examinations, to inform care plans.
Administer medications, injections—including IV infusions—and treatments in accordance with physician orders and safety protocols.
Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
Support patients with activities of daily living and provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments.
Manage complex cases involving acute pain management, ventilator support, catheterization, and wound care using aseptic techniques.
Maintain accurate medical documentation within electronic health record (EHR) systems such as Epic or Cerner while ensuring compliance with HIPAA and confidentiality policies.
Collaborate effectively with interdisciplinary teams to coordinate discharge planning, case management, and long-term care strategies.
Provide education to patients and their families regarding medication administration, health coaching, and post-discharge instructions.
Respond promptly to urgent situations including triage assessments in emergency or urgent care settings.
Uphold infection control standards and adhere to long-term care regulations across all patient interactions.

Qualifications

Valid state licensure as a Licensed Practical Nurse (LPN)
Prior experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, skilled nursing facilities, or home & community care settings.
Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; experience with ICD coding (ICD-9/10) is preferred.
Demonstrated skills in IV insertion, phlebotomy, medication administration—including injections—and sterile processing techniques.
Knowledge of physiology, anatomy, pharmacology related to pain management and acute care procedures.
Strong communication skills for effective caregiver-patient interactions and team collaboration.
Ability to handle complex cases involving infectious disease care or specialized populations such as pediatric or neonatal patients.
Certification in Basic Life Support (BLS) or CPR is required; additional certifications in first aid or advanced life support are advantageous.
Experience working with individuals with disabilities or developmental challenges is highly desirable.
