Job Overview
The Clinical Pharmacist provides direct customer-owner-centered care and integrated pharmacy operational services in an interdisciplinary outpatient setting with physicians, nurses, and other allied health professional at the Little Shell Health Clinic (LSHC). The Clinical Pharmacist will participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the customer-owners in the LSHC service area. Pharmaceutical care services include but are not limited to assessing customer-owner needs, incorporating age and disease specific characteristics into drug therapy and customer-owner education, adjusting care according to customer-owner response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.
Responsibilities
- Facilitate pharmaceutical care services, direct customer-owner care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with customer-owner care needs, evidence-based best practices, and regulatory standards.
- Design and implement stewardship activities and restriction/surveillance programs.
- Track and evaluate assigned pharmacy programs for operational, quality, and financial efficiency and routinely benchmark against local and national best practices.
- Actively identify practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice.
- Develop and oversee policies and procedures for drug purchasing, drug usage, drug distribution, and drug control.
- Perform comprehensive medication reviews.
- Assure pharmacy is an integral part of the health-care delivery system and facilitate enhancement and expansion of pharmacy services/programs.
- Deliver direct customer-owner care and clinical practice, including decentralized and service-based programs.
- Maintain proficiency in decentralized pharmacy services and clinical pharmacy programs.
- Work as an active member of multidisciplinary team and collaborate with healthcare providers within decentralized customer-owner care areas to provide customer-owner-centered care.
- Identify high-risk customer-owners and implement interventions to improve quality and safety.
- Make appropriate evidence-based, customer-owner-centered medication recommendations.
- Provide discharge medication review, reconciliation, and counseling as appropriate.
- Complete critical customer-owner monitoring and review customer-owner profiles/charts to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.
- Facilitate specialty medication procurement, ordering, and dispensing procedures including but not limited to chemotherapy, controlled substances, etc. as appropriate.
- Provide exceptional customer service and model the mission of the LSHC when interacting with internal and external stakeholders, promoting a positive image for the health clinic.
- Other duties as assigned.
Training & Skills
Must possess at least four years’ experience as a Clinical Pharmacist. Experience working with American Indian/Alaska Native, minority, or under-resourced communities is desired. Experience working with or for an Indian Health Care Provider would be preferred.
Knowledge
- Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine.
- Strong knowledge of emerging medications and relevant research.
- Strong understanding of legal regulations for prescription drugs.
Skills
- Proficiency in the use of EHR and pharmacy software.
- Skill in relaying complex care instructions and education with empathy to customer-owners, care teams, and community members.
Abilities
- Ability to monitor the quality of work performed by all levels of nursing staff and develop performance standards.
- Ability to evaluate and improve clinical workflows to enhance productivity and reduce costs.
- Maintain confidentiality and privacy with all aspects of information in accordance with practice, State and Federal regulations.
Qualifications
- Graduate of an accredited College of Pharmacy.
- Doctor of Pharmacy preferred.
- Requires a current license to practice as a registered Clinical Pharmacist.
- Maintain current BLS (CPR and AED) and DEA license for Healthcare Providers certification.
Physical Demands
Essential job duties may require regular lifting, pushing and pulling up to 10-20 pounds, and the ability to sit for long periods. The ability to hear normal conversational discussion and near and far visual clarity is required.
Conditions of Employment
- Must pass a pre-employment criminal and Department of Justice background check.
- Candidates must possess a valid driver’s license, no alcohol related conviction, and meet the insurability requirements of the Tribe.

About the Little Shell Health Clinic
The Clinic provides outpatient services to American Indian and Alaska Native citizens including, but not limited to, family medicine, pediatric care, behavioral health, laboratory services, pharmacy, radiology, optometry, nutrition services and dental care. The Clinic currently operates Monday-Friday, 8:00 am to 5:00 pm.
