That's the kind of practice this is.
A mission-driven community health organization in Central California is seeking a Family Nurse Practitioner or Physician Assistant to join a primary care team that has been embedded in its community for decades. The panel is yours to build and grow. The relationships are long-term. The work is meaningful in a way that high-turnover, high-volume systems rarely allow.
This is a permanent, non-travel position. The organization has been here a long time and so have its providers — that's not an accident.
The Role
Full-spectrum primary care across the lifespan — pediatrics through geriatrics. Well-child visits, adult chronic disease management, same-day acute care, and everything in between. Two dedicated MAs support each provider, a daily admin block is built into the schedule, and the salary is straight — no RVU calculations, no productivity pressure, no end-of-month anxiety about your numbers.
- Schedule: 4x10s with a rotating Saturday (when Saturday is worked, Monday is typically off)
- Volume: Up to 22–24 patients per day — scheduled and same-day walk-ins
- Appointment slots: 20 or 40 minutes
- Support: Two dedicated Medical Assistants per provider
- Admin time: 1-hour lunch and 1-hour daily admin block built into the schedule
- Compensation: Straight salary — no RVU, no productivity pressure
- CME reimbursement: ~$3,000/year
- Controlled substances: Schedule II prescribing per your comfort level — not required
What You'll Be Doing
- Managing a broad panel including pediatric well-child visits, adult chronic disease — diabetes, hypertension, medication management — and same-day acute needs
- Building and growing your own patient panel over time
- Collaborating with an on-site multidisciplinary team including pain management, orthopedics, general surgery, and pediatrics
- Working with robust language access support — all support staff are trained medical interpreters; in-person, telephonic, and indigenous language interpretation also available
Support Structure
- Physician on-site in the building every weekday
- Structured mentoring program for newer providers — scheduled check-ins with supervising physician and midlevel peers 1–2x/month
- Experienced providers work with significant autonomy
- Collaborative culture — consult colleagues down the hall as needed
What You Need
- FNP or PA-C credentials — required
- Active California NP or PA license — required prior to start
- Active DEA registration — required
- Minimum 3 years of primary care or family medicine experience — required
- Comfort with full-spectrum care: pediatrics through adult and geriatric chronic disease — required
- Commitment of 2+ years strongly preferred — continuity of care is central to how this organization operates
- Bilingual English/Spanish — a genuine plus
- Not a fit: occupational medicine only, behavioral health only, or orthopedics only without meaningful primary care experience

About an hour inland from the Monterey Bay coastline — agricultural valleys, open country, and a community with real civic character. The Monterey Peninsula, Carmel, Paso Robles wine country, and Big Sur are all within comfortable driving distance for weekends. The cost of living is lower than coastal California, the commute doesn't exist, and the people who work here will tell you straight what daily life actually looks like — we encourage you to ask them.
Out-of-state applicants are welcome. California licensure typically takes 1–3 months; credentialing 60–90 days.
