Full Job Description

WORK ENVIRONMENT: Hybrid (2 days remote; remaining days in the office). Ability to work evenings.

JOB SUMMARY: Under the supervision of an Intact/OCC Supervisor, provides case management, family support, assessments, and referral services to children and families participating in Intact Family Services and Opportunity for Change (OCC) services. Responsibilities include conducting home visits, completing required assessments, coordinating services, connecting families with community resources, and promoting child safety, family stability, and permanency. Services are provided in family homes, community settings, and office environments.

KEY RESULT AREAS:

Intact Responsibilities

· Complete all DCFS-required assessments, visits, and documentation within established timeframes.

· Develop and monitor service plans with families.

· Conduct home visits and assess child safety and family functioning.

· Coordinate services and referrals to meet identified family needs.

· Prepare court reports, progress reports, and other required documentation.

· Participate in case staffing's, supervision, court hearings, and Child and Family Team Meetings.

· Maintain accurate and timely case records in accordance with DCFS and agency requirements.

OCC responsibilities

· Conduct assigned families within the required DCFS timeframes

· Complete Multiple Family Service Assessments (MFSA) and other required OCC documentation.

· Assess family strengths, needs, barriers, and protective factors.

· Provide education related to child safety, parenting skills, and family stability.

· Connect families to community resources and supportive services.

· Conduct outreach efforts to engage families and maintain participation.

· Complete supervisory consultations and closure documentation as required.

· Monitor progress toward service goals and successful case closure.

General Responsibilities

· Maintain confidentiality in accordance with HIPAA, DCFS, and agency policies.

· Participate in required training and professional development activities.

· Collaborate with internal and external service providers.

· Respond appropriately to crisis situations and safety concerns.

· Perform other duties as assigned.

EDUCATION: Bachelor's degree in social work, psychology, or other human service fields.

EXPERIENCE:

· Minimum of two years of experience in child welfare, family services, social services, or related experience preferred.

· Experience working with at-risk children and families preferred.

· Knowledge of DCFS policies and procedures preferred.

LICENSES & CERTIFICATIONS:

Valid Illinois driver's license
Ability to comply with agency insurance requirements if using a personal vehicle for work purposes.
Ability to meet DCFS training and credentialing requirements.
Successful completion of all required DCFS child welfare training

PHYSICAL DEMANDS: The physical demands described are representative of those an employee must meet to successfully perform the essential functions of this job. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.

· Ability to sit for extended periods while working at a computer

· Ability to lift or transport light materials (up to ~20 pounds) occasionally

· Frequent driving throughout assigned areas

· Ability to conduct home visits in various environments

SUPERVISORY RESPONSIBILITIES: None.

JOB REQUIREMENTS:

· Bilingual English/Spanish preferred

· Strong organizational and documentation skills

· Excellent written and verbal communication skills

· Ability to work independently and manage multiple tasks.

· Strong assessment and engagement skills

· Knowledge of community resources and service system preferred

· Ability to work with families experiencing crisis, trauma, poverty, substance abuse, domestic violence, mental health concerns, and other barriers.

Pay: Up to $67,500.00 per year
